introducing my OCs: the rabbit and the goat-man!!

[CW for torture, captivity, kidnapping, written and visual depictions of injury, mentions of amputation/decapitation, noncon touching, very vague mentions of sexual content?]

(also warning this post is LONG :P)

very basic info and initial designs

these two OCs started out on a random whim one summer’s night in 2021, and from then I’ve tried to tone down the amount of lore I’d add to them (because I REALLY love unnecessary worldbuilding) which I kind of failed at so sorry if this post is long.

about the rabbit:

- he isn’t actually a rabbit, as a matter of fact he’s a strange immortal being with the features of a large hare, a fox-like tail, slit snake pupils, canine teeth, and makes cat noises. quite the conglomerate of creatures indeed, but the goat-man just calls him a rabbit, and so that is his name.

- whenever a body part or organ is completely removed from the rabbit, a new and perfect replica grows in its place within typically a couple hours, although it depends: a few missing fingers or teeth only takes an hour or two whilst a removed spine or decapitated head could take well over a day. however, the removed body part remains for a short period of time before quickly decomposing (although if preserved properly it will remain fresh and not break down). the exception is the head and more specifically the brain, which both disintegrate immediately upon a new head/brain growing into place and cannot be preserved.

- anesthesia and other painkillers don’t work on him :3 muscle relaxants do immobilize him enough for the goat-man to operate on him, though…

- and did I mention he can’t pass out or vomit? he can’t be knocked unconscious unless he falls asleep on his own, and his stomach forcibly turns any and all nutrients, poisons and toxins into usable energy regardless of how painful the process is. I fucking love making this guy miserable

- his nerves/touch receptors are also more sensitive than a human’s, but on the bright side his immune system is strong enough to not just regenerate literally everything but also prevents infections, inflammations, rotting flesh, etc

- he can turn into a big fluffy hare looking thing!!! (see flemish giant rabbits for an approximate reference, although he’s bigger— the size of a large dog— and looks somewhat different). the goat-man finds this version of him irresistibly cute so he doesn’t really abuse him in that form, although he can forcibly shapeshift the rabbit back using intense electric shocks. I’ll link some art of what his animal form looks like later!

about the goat-man:

- unlike the rabbit he does look like a man crossed with a goat (although with an extraordinarily long tail), but it seems like he’s effectively immortal as well, it’s strange.

- an out-of-practice general surgeon, he technically does specialize some in orthopedics and a bit of thoracic surgery. he doesn’t operate or come to the hospital anymore though, instead choosing to bring hell upon the poor rabbit instead.

- when not covered in the scent of blood and gore, the goat-man smells of lemongrass and sandalwood and is actually pretty comfortable to sleep on/with. yeah.

- unexpectedly became obsessed with the rabbit’s fluffy ears and tail and pets them every chance he gets, and is positively in love with his animal form.

- self proclaimed mad scientist like the looney he is, sociopath, clean freak, taxidermy enthusiast, ex-satanist and sadist x100 :)

about both of em:

- they’re probably both aro/ace, and the goat-man has no libido whatsoever. the rabbit on the other hand, being a lagomorph hybrid… yeah I might not discuss it too much (we’ll see) but things do happen with him lol

- neither of em are good people. there’s not much else to be revealed about here (again I’m trying to cut down on lore) but all you have to know is that the goat-man is pretty explicitly… a sadistic shit and the rabbit actually used to be quite a malevolent force too in his past life. his backstory isn’t discussed about currently besides some passing mentions, he’s a little too broken for any of that anymore.

- because the goat-man is also the caretaker and comforter as well as the tormentor, the rabbit has developed an almost borderline stockholm syndrome towards him, referring to the goat-man as “sir” or occasionally “doctor” out of terrified politeness (ok tbf he doesn’t know his name) and has a habit of burying his face into his captor’s neck when he is allowed to sleep on the bed ;w; (it doesn’t help that the goat-man is his only source of human interaction and therefore he must rely on him for everything.)

- their little shenanigans happen almost like in a bubble inside the goat-man’s strange house in the middle of an infinitely vast, silent woods. there’s very strange things happening here indeed, and a (not confirmed canon) theory that not everything is as it seems… sigh just when I said I would cut down on the lore

- will the rabbit ever be able to take his revenge one day? maybe in an alternate universe… I have a chart planned of all the possible AU versions that’ll be revealed later haha

I BELIEVE UNIVERSAL MAKING IS A HORRIBLE IDEA

here is why

Math. I’m going to explain why universal masking is a horrible idea with math.

The problem of universal masking has been so clear to me that I haven’t engaged the debate with any presence. The arguments fly back and forth with equal parts passion and incredulity. “How can you think that, you monster” is the conclusion of any “dialogue”. Let’s face it, there is not much dialogue happening on this planet right now in any arena.

So that’s it! I’m stepping it! I’m going to put this to rest once and for all and I am sure my massive following and millions of readers will usher in a new age of sound reason.

Math.

First a couple of points to clarify mask use before I astound you with the most obvious of equations.

Masks do work, at some things. But not all masks work equally at all things. Dr. Fauci the magnanimous just stated what matters is wearing a mask, any mask. This type of ridiculous logic is why I am spending time earmarked for Riverdale to write this post. This statement is, in a word, stupid. It is meant to give the panicked public a sense of control, but it also creates a religious cast of acolytes, and by nature of the apparent consequences, self appointed inquisitors. It’s garbage science to be sure, but it is even worse leadership. Even the once holy Michael Osterholm stated that cloth masks are ineffective (August 3, 2021). And they mostly are ineffective for anything positive besides a fashion statement. (I don’t take blind adherence to be positive). So let’s just take these ridiculous and useless items out of our discussion. They are not for the serious inquirer.

What about the surgical masks? Medical personal wear them! Surely, they do this for a very good reason. Why do they wear masks in the operating room?

This line of thought is again, so obvious to me I believed it would have been discussed ages ago. These masks are effective for droplets. Period. And why are droplets important in sterile environments? Well it’s the word sterile. Bacteria people. Nurses generally wear the masks to stop blood or puss or any manner of ungodly particulate flying in their nose and mouth. It is a thin veil of self protection and I have nothing but respect of those that put themselves in the presence of bacterial harm with just the simplest of PPE. In the operating room surgical staff wear those masks for the same purpose. Bacteria. First to stop a spurting vein or a pustule explosion from entering their orifices. Also to stop droplets from going from their mouths into an open wound. The human mouth has almost as much bacteria as a dog’s butt. Stopping a bit of spittle entering the human envelop is a rather important thing to do. These masks are quite effective at this. Droplets are of a size that surgical masks will stop them from travelling into the wound. Note that these are droplets not aerosolized particles.No serious medical person will say that the masks stop viruses. Not one. (Seems that there are many who are no longer serious. Or maybe they are so serious they have lost their minds.)

Viruses are too small to be stopped by these materials. You may argue that the viral particlescan be diverted by the pressure of the mask to escape to the sides of the face and therefore lessen the potential viral load hitting the open wound before the surgeon. But that would be silly. Any aerosolized virus would hang in the air and settle on everything in the room. These masks don’t stop viruses. Says it right on the box just in case the surgeon forgot their first year of medical school. The only solution to stop a surgeon from giving his virus to a patient is that the surgeon call in sick when feeling unwell. If these masks don’t stop viruses in a controlled sterile environment, they won’t do anything in the wider world either. But what about N95 masks? Let’s talk about them and then explain the real reason universal masks are a terrible idea.

There is a case for N95 masks. They appear to be effective in stopping viral particles to a degree. That amount has not been well tested, but I am going to give the mask brigade the benefit of the doubt on this one. An N95 mask that is properly fitted to the face and left alone during its wear may have an effect. (For the purposes of this post I will say it will have an effect because the reason universal masking is a bad idea has nothing to do with effectiveness). Now please note that the mask must be properly fitted. Buying them in bulk from your local shop won’t do. You need a proper PPE fitting and once you put that sucker on you cannot touch it. No drinks. Sorry. No smokes outside on your break. No food. None of that unless you are willing to discard the mask and get a new one. Remember we are wearing these things to protect everyone else from us as the wearer. So, the protocols for use are far more stringent. You must never break the seal of the mask when in use. Ever.

I will grant for the purposes of this exercise that N95 masks are effective if properly fitted and worn properly without tampering. And that leads me to the absolutely obvious reason they are a terrible idea.

Math.

We don’t discriminate right? Everyone should be treated equally, right? Every man woman and child have the right to health, right? And more importantly, according to our health overlords every man woman and child is a breeding ground for viral death. Therefore, every last one of us on the planet MUST wear a N95 mask. Every one of us. No exceptions. And every one of us must wear these masks properly. And these masks must be changed whenever contaminated. Period. That’s the science folks. So we need 7.6 billion masks a day to fully cover the planet. But let’s assume that some of the people need more than one because they have active jobs. It is only logical. 10 billion masks a day sounds like a nice round number. That’s what we need. 10 billion. The cost will be rather large. Even if we get the crappiest Chinese knock off company to make them for 50 cents a mask (quality I’m sure), that is still 5 billion dollars a day spent on masks. That will cost One trillion eight hundred and twenty-five billion dollars a year. A small price to pay to save a couple of lives right! I mean we can’t feed the hundreds of millions of people starving to death right now but let’s get them all a mask. Worth it! Logistically this may be a challenge. We will have to fully occupy the global supply chain to deliver masks every day to spec so let’s wave goodbye to all other imports. That probably includes food but if we all have masks at least we won’t die of a virus. And don’t worry about the environmental impact. We banned plastic straws, so I’m certain the massive increase in biohazardous materials made of…. Drum roll… petroleum won’t be an issue. Oh ya, didn’t you know that these masks are made with a large amount of petroleum products? Don’t worry the planet understands. The dinosaurs died and decomposed so we can discover the glory of… universal masking.

Folks, it’s not a bad idea. It is a ludicrous laughable idea that is blocking the path to real solutions. Do the math. Mask people in high impact places where the protocols can be followed. And move on.

Declaration of Unity - A declaration read and signed by Gabrielle Adawe during the founding of Numbani.

Delivery drone - Used to deliver things. Some are powerful enough to carry an 11-year-old.

eNaira - Electronic Nigerian currency.

FacePunch - Site where Efi finds a lot of jokes and memes, such as Marley the Dancing Coconut.

Fadeout/Fading out - An omnic condition induced by a malicious code that self-replicates and overwrites the omnic’s native functions one-by-one. The name comes from the lights on an omnic’s head fading out when it is affected by this condition. Omnics experience severe program malfunctions during fadeout. For example, an omnic forklift operator might keep driving the forklift into a wall over and over again, or an omnic artist might destroy her own work. Fadeout also causes an omnic’s private wireless port to become publicly accessible, and if the malicious code is allowed to operate for long enough the omnic can lose all original programming and memory. Fadeouts were induced in Numbani by Sombra for a time after Doomfist’s attack on the airport. Because fadeout causes omnics’ wireless ports to become publicly accessible, an “antidote” code can be administered to one omnic and will be automatically distributed to all other affected omnics in the area.

Ipanema - Real-life neighborhood in Rio. Efi planned to take surf lessons here, and Lúcio’s studio is here.

Junker reactor - A graviton reactor purchased by Orisa when a miniature Tobelstein reactor cannot be found. Cobbled together from mismatched parts, with some graffiti on the side. Efi notes that it is probably irradiated and likely to blow up in Orisa’s face.

Junie (Junior Assistant) - A robot stand-in for social and professional situations, capable of broadcasting holograms of people and recording video/audio with a 360-degree camera. Hologram features can be upgraded with hardlight conversion kits. Invented by Efi Oladele. Holograms produced by Junies are referred to as [name-of-the-person-being-shown]-Junior. For example, Naade-Junior for a hologram of Efi’s friend Naade.

Lagos - The largest real-life city in Nigeria, on the coast.

Maxwell Interpreter - Part of Efi’s robot creation kit. Used for creating simulations by autosorting “eight billion permutations into virtual hash matrices.”

Modulated Biochemical Currency (MBC) - DNA-coded cryptocurrency used on the dark web. About the size of a sand dollar, they wiggle in your hand and contain synthetic blood with unique biological codes in it. Also known as bio gold, wiggle notes, glam clams, scritch scratch...

Nollywood - Real-life slang for the Nigerian film industry

Numbani Omnic Union - An omnic representative organization present during the founding of Numbani. The leader was presented the Harmony Key by Gabrielle Adawe.

OmnicCon - A con in Numbani with a costuming contest.

OmniWorx - Company that produces synthetic greases in designer tins with different scents like mint and citrus. The product is used by omnics as an indulgence, such as on Unity Day in Numbani. Efi buys OmniWorx tins for her omnic friends.

OR15 auction - An auction that took place after the defeat of the OR15s by Doomfist at Numbani airport. The Numbani Civic Defense Department sold off decommissioned OR15s in lots of 10 that cost around 20 million naira. Bidders were not informed of the OR15′s defeat at Doomfist’s hands.

Overwatch cartoons - Cartoons Efi describes as “old” that are based on the lives of real Overwatch members like Sojourn.

Overwatch coin bank - Owned by Efi. Features Reinhardt protecting the bank with his rocket hammer.

Overwatch pajamas - Efi owns a pair of these.

Paper notes - Used to avoid digital filters at Efi’s school.

Peace Park - A park in Numbani which had a statue of Gabrielle Adawe. The statue ended up being destroyed by Doomfist.

Precision Core reactor - Alternative to the miniature Tobelstein reactor. Unit is the size of a car.

Rapid-X heliotherapy - A type of therapy that heals broken bones in the span of a few days.

Sky Postal - Delivery company that uses drones. Capable of executing Lightning-Priority deliveries.

Super Fun Family Time - A “game” Efi’s parents taught her where they hide in the interior bathroom of their flat whenever the warning sirens go off indicating a Doomfist attack.

Tiawo Boulevard - Street in Numbani that intersects Heritage Avenue

Tin Can Island Port - Port where Numbani omnics held a dockworkers’ revolt several years back.

Tonal Abyss - Omnic pop band with 38 members. Broke up at one point due to disagreements about which quantum clock standard to use as a time measure for their music. Reunited at Lúcio’s concert in Numbani.

University of Ibadan, University of Lagos - Real-life Nigerian universities that Efi’s cousin Dayo received acceptances from in the story.

Valor Matrix distributed computing project - Project that rewards loans of processing power with currency.

Version 3.44 VAvmpCompiler - Part of Efi’s robot creation kit. An alternative to a dedicated Maxwell Interpreter box.

Virtual Physician protocol - Downloadable physician protocol accessible to any robot or omnic.

Yankari National Park - Largest national park in Nigeria.

Yoku Voyager - A low-end car model with weak levrims.

Zobo Bot - Robot that makes juice drinks at Efi’s school. Known to be ornery.

Kiba

v; always in the doghouse Kiba Inuzuka comes from a long line of dog breeders and veterinarians and grew up living alongside his family’s animal shelter. He spent his youth learning how to care for animals of all kinds, and his friends were the numerous canine guests taken in over the years. His loyal companion, Akamaru, was a rescue brought to the shelter as a puppy, who bonded instantly with Kiba. As an adult, he runs the Inuzuka Animal Shelter alongside his older sister Hana, the head veterinarian.

Itachi

v; the light that casts a shadow Itachi Uchiha is a genius. That’s what he’s been told his entire life, forever forced to live up to the high expectations bestowed upon him by family, teachers and friends alike. All Itachi wants is to live his life how he desires, to chase his own dreams, and to let his younger brother claim some of the limelight for himself as he deserves.

Hidan

v; definitely not an angel Every parent’s nightmare, Hidan Noroi was branded a problem child from a young age. Passed from foster family to foster family, nothing was ever the right fit for a child like him. Out of the system at eighteen and unleashed on the unsuspecting world, Hidan doesn’t exactly seek out trouble, but it finds him anyway. He never sticks around in one place for too long, always on the move, and usually avoiding the authorities for some reason or another.

Kakuzu

v; a heart for the work Kakuzu Sutetchi is a renowned cardiothoracic surgeon, one of the best in his field, and notoriously antisocial. Some believe him to be arrogant and egotistical, but the truth is he simply prefers his own company. He went into medicine after his mother’s death when he was still a teenager, developing an almost obsessive need to succeed which, inevitably, resulted in his fame. An incident in his late teens left him with awful scarring across his cheeks and it is only in the operating room, hidden beneath the surgical masks, that he can feel human again.

Madara

v; the weight of dreams and legacy Madara Uchiha never wanted to head the family business, but when his only remaining brother dies young, it seems his fate is sealed. He cares little for that world: he’s always had such big dreams for the future, and none of them involve pandering to the socialites and politicians who would rather see the world starve than let go of a few pennies from their vast wealth. Of course, having such influence over the rich and powerful could come in handy…

Yamato

v; a talent for starting from scratch Yamato Senju is a highly successful architect, responsible for a lot of the famous buildings on the Konoha skyline. Orphaned young, he cannot remember ever having a home that was solely his, thus sparking a dream to one day build one with his own hands. This dream would be realised when, as an adult, he builds his perfect home from his own design.

Kotetsu

v; born a reckless spirit Kotetsu Hagane has coasted through life carefree and content, seemingly never taking anything seriously, much to the despair of his lifelong friend Izumo. He can’t seem to hold down a single job for very long, growing bored of the monotonous day-to-day and seeking something far more exciting. He talks of travelling the world, exploring wild jungles and deserts, seeking adventure. Whether or not he’ll ever do it, however…

- ahhhhhhh confirmation on the Jonah!Elias theory at last. and Elias and Peter can apparently bond over enjoying startling Martin.

- wow I cannot wait to find out how this happened. It sounds like Jonah’s eyes are in Elias’ body and that’s how the possession works, so - I don’t imagine they involved a surgeon? mechanically, how did they do that? was it like Operation?

- and this is Gertrude’s last stand, apparently - wow we are getting so much this episode. I hope this won’t be an end to hearing her statements - it feels like we still have a lot to learn from that era.

- I adore the way she’s saying Elias’ name right now, it’s so stabby

- I thought she was shot more than that? According to the transcript for Human Remains, she was shot three times, so... when did that happen, if it wasn’t then? did Elias decide later, “you know what, I’m still pissed, I’m going to go and do some overkill”?

- I really hope Martin does care about them still, somewhere. Or at least that the caring can be stapled back in at some point. I’m rather fond of the caring. I believe I’ve said before.

- Peter, did you read a book on villain speeches? “we’re the same” I swear. I guess being Lonely he’s not had much practice at them? but you’d think he could have put some work in with Martin, and Brian, and that.

- “I think I would” okay I am putting a lot of my emotions into that would, it’s a hypothetical not a certainty, come on Martin, don’t do it - YES WELL DONE GOOD BOY

- oh no the poor Institute staff! I hope Rosie’s okay, and Sonja, I’m not sure we have many other names but honestly, Trevor and Julia that is just rude.

- “YOU NEVER TAUGHT ME” I really was not expecting to laugh out loud in this episode but here were are I guess.

...I actually did forget about this, haha. Here’s the scene in Throne of Jade, from Granby’s POV, when Laurence fells unconscious/was confronted by Barham etc. Some dialogue in the middle is taken directly from the text.

In 1801 Granby had been freshly promoted to lieutenant when Fluitare's captain, Sommer, was shot in the shoulder during a bout.

He still remembers the whole crew surging forward, the ferocity of first-lieutenant Jonson as the man flung away his carabiners to run pell-mell down Fluitare's back, stumbling in the fierce wind. If Fluitare had reacted to the shouts, twisting to look around as anxious dragons often did, the lieutenant would have fallen. But Fluitare was a veteran and flew steady. Jonson ended up bodily launching himself at a Frenchman and throwing the man, kicking and flailing, over the dragon's shoulder and down toward the sea below.

Captain Sommer thanked the lieutenant, then called him a damn idiot and ordered him to retrieve his carabiners.

It is of course the primary duty of every aviator to protect their captain. A dead captain might cause a dragon to go wild with grief; an unconscious captain will make a beast panic. And afterward a dragon torn with despair may well reject all company, and thereafter be sent straight to the breeding grounds for a dull and useless life of misery.

But whenever Captain Laurence is threatened Granby never thinks of Temeraire – he remembers that desperate charge on Fluitare, the flung carabiners. He thinks of this now when he sees a Frenchman cut himself free from Temeraire's harness, lunging toward Laurence in a cruel parody of lieutenant Jonson's selfless action.

Granby struggles in the direction of Temeraire's neck, where Laurence has drawn his sword to engage the Frenchman. But there is no room here for a bold charge; there are too many boarders, and Temeraire's crew is composed greatly of young officers. Granby fumbles for a pistol with his left hand and shoots almost distractedly when he sees a man threatening young Roland with a sword, holding his own blade defensively with his free hand. He has no attention to spare for the rest of the fighting.

Up front Laurence stumbles, staring at his weapon in shock. A chunk of it is missing, and, blast – is the man fighting with a dress-sword?

Granby unlatches his carabiners.

“You damn idiot,” he hears Captain Sommer scold. Laurence will probably disapprove, too. He may chastise Granby all he wants if survives.

Granby reaches the front in time to cut down a rifleman flanking the French lieutenant, shoving the body over Temeraire's side. Behind Granby six bellman swarm forward onto the top, threatening the other boarders. Granby didn't realize anyone was behind him.

The lieutenant up front makes a last, desperate attempt to capture Laurence before being knocked out by young Digby. Granby finally lets himself sheathe his sword, pushing forward to check Laurence, when suddenly the world seems to flip sideways.

Men shout as a huge body crashes into Temeraire – a Pecheur making a rash bid to support the boarders. One of the French wingmen, unfortunately in the midst of repositioning, slides away soundlessly and disappears into the air. Granby finds himself on his knees clinging desperately to the harness-straps. He stumbles to his feet seconds later and looks up.

Laurence lies limp on Temeraire's neck, body sliding from one side to the next as Temeraire readjusts. Only the carabiners hold him in place, and even from a distance Granby can see a bloom of blood spreading about his collar, one leg bent awkwardly in the straps.

“Temeraire!” he shouts. The dragon's head twitches as Temeraire swerves, evidently positioning himself to tackle the Pecheur. “Temeraire, Laurence has been hurt!”

This, as intended, distracts the dragon from his outrage. “Hurt?”

“We must return to the covert!” Granby shouts. He can see Keynes working toward the front, but waves the man forward anyway, uselessly. “We must return now, do you understand? You must disengage.”

Temeraire drops suddenly to avoid another pass from the Pecheur, angling himself at once toward the shore. Behind them the Pecheur slows as though considering its course; then it wheels away, flying quickly toward the other fighting dragons to lend its support.

Granby reaches Laurence at last in the company of Keynes. The dragon-surgeon tsks under his breath, nudging Laurence's head from one side to the other. “He was crushed when the dragon knocked into Temeraire; hit his head, it seems. And I don't like the look of that leg. I will tell the men to retrieve a stretcher when we land, but I must fetch a few supplies. I don't plan for human injuries, you know.”

Temeraire lands in his clearing with great haste, craning his neck around as soon as his legs hit the ground. “Well?” the dragon demands. “Is he well?”

Before Granby can respond, though, they are interrupted.

Somehow in all the fuss Granby did not even notice the unusual number of people as they descended into Temeraire's clearing; did not note the splash of red and blue coats among the crowd. Now one man shoves himself forward, carrying a gun; he brandishes it up at Temeraire's outraged face and seems pitifully small. “You damn beast!” the man roars. A navy admiral. Why the hell is a navy admiral in the covert, Granby thinks, and abruptly remembers the idiotic mess with the Chinese. Temeraire is not supposed to be here. “You will not move, you will not fly from this spot - Captain Laurence! Captain Laurence, if you do not present yourself immediately, you will be charged not just with this blatant insuburdination, but for conspiracy to mutiny - “

“You will not charge Laurence with anything,” Temeraire bristles.

“Oh, hell,” Granby says. “Keynes, go get your bandages and whatever else, and do it quickly. Baylesworth, find a stretcher for the captain. Mr. Riggs, Mr. Martin, with me! Roland, go find Lenton, hurry now - “

Admiral Barham seems only more furious when Granby descends to meet with him. “I swear to you,” he begins, “If Captain Laurence resists arrest we shall use force - “

“Good god, man, shut up,” Granby snaps. “Captain Laurence is injured, and you must wait to see him - “

But the damage is already done. Temeraire snarls and lowers his head above Granby, his blood-smeared ruff flaring wide.

The admiral takes a few steps back, face mottling red. He roars, “You will control that beast, lieutenant! All of you will stand down!” To his men: “Retrieve Captain Laurence!”

“No, sir,” Granby snaps. These idiots are likely to get everyone killed if they try to reach Laurence, and the last thing he needs is to be dragged half-lucid before the admiralty. “No farther, not one damned step. Temeraire, if these men make ready you may knock them down.”

He says this because he fears Temeraire will do worse, if they restrain him. Not that he would blame the dragon – Granby is ready to shoot Barham himself.

And certainly the stupid man is prepared to tempt him. Granby cannot understand how Laurence came from the same background that shaped this flailing, spitting farce of an officer. Laurence matches this man only in sheer bull-headed stubbornness – a trait Granby despairs when he notices Laurence, white-faced and heavily supported by Digby and Allen, limping toward the group.

Barham sees him, too, but evidently dismisses the obvious injuries. “There you are; did you think you could hide here, like a coward? Stand down that animal, at once; Sergeant, go there and take him."

"You are not to come anywhere near Laurence, at all," Temeraire snarls. The dragon raises a clawed leg. Granby has seen such postures from dragons – has seen a usually mellow Pascal's Blue kill a man for just insulting her captain, much less threaten him. If the streaks of blood around Temeraire's head do not instill Barham with proper wariness than the man deserves his fate.

But Granby is more concerned about Laurence. The captain sways on his feet despite the support of Digby and Allen. As Temeraire argues with Laurence – making it plain that he will allow no one to arrest his captain, or execute him, or any such thing – Granby impulsively gestures to the other crewmen.

He feels a surge of pride when they respond immediately. Ferris, Evans, Riggs and all the rifleman line up in front of Temeraire with their guns aimed at Barham's marines. Granby keeps his own rifle trained straight at Barham. Temeraire is more of a threat than any of his officers, of course, but Granby would blush if he made an injured dragon defend himself against these morons.

And quite frankly, he is itching to put a bullet in the admiral.

The marines hurry to raise their own weapons, several of them fussing over the feeble cannon that they apparently thought could be used to menace a heavy-weight dragon. Granby's fingers tighten around the gun. Barham shows no signs of backing down, and Granby is seconds away from giving the order when suddenly their stand-off is interrupted.

"What the devil do you all mean here?" Keynes shouts. He shoves between the confused marines, snatching a match from the man standing to light the cannon.

Granby slowly lowers his gun as the surgeon berates them. The rest of the men follow his lead. “He is fresh from the field; have you all taken leave of your senses? ….Out, out at once, the lot of you; I cannot be operating in the middle of this circus, and as for you, lie down again at once; I gave orders you were to be taken straight to the surgeons. Christ only knows what you are doing to that leg, hopping about on it. Where is Baylesworth with that stretcher?"

Granby is grateful that his hand is far from the trigger when Barham replies. “Laurence is damned well under arrest, and I have a mind to clap the rest of you mutinous dogs into irons also - “

Keynes shakes his hook-tipped fist in the admiral's face, startling him into silence. “You can arrest him in the morning, after that leg has been seen to, and his dragon. Of all the blackguardly, unchristian notions, storming in on wounded men and beasts - “

The haranguing serves its purpose, at least. Barham does leave, fury evident in every step. But Laurence is supremely distressed - more distressed than the mere injury would warrant, for Granby deeply suspects that Laurence is not concerned about his broken leg at all.

The crisis over, Keynes attends to Temeraire's leg while Laurence is brought, unwillingly, to the sickroom for his leg to be scrutinized. He makes matters more difficult by disdaining the stretcher, though honestly Granby expected that. Mr. Evans, being abnormally tall, helps support him and keep the man from mangling his body further.

The surgeons seem puzzled by the captain's injuries, which does not please Granby; Laurence also refuses an offer of laudanum, which is almost worse. The captain is left with strict instructions to not put weight on his leg, after which he asks Granby for assistance to his rooms.

Granby walks him there despite the Laurence's shaking, and pretends not to notice when Laurence has to stop, halfway up the front stairs of the building, to brace himself against the wall. When they arrive at his quarters Laurence throws open the door and limps inside, plainly expecting him to follow.

Granby closes the door.

“I am sorry that you have all been caught in this mess,” says Laurence. Granby, who only expected a rebuke for his earlier insubordination, is caught off-guard. Laurence looks infinitely weary. “First the Chinese, and now this... I assure you that I will approach Admiral Lenton to plead for leniency on the crew's behalf. I cannot condone your actions, Lieutenant; you ought not have contradicted Barham's orders, and I am sure you know it. But I will try to explain the circumstances.”

Granby shakes his head. Laurence does not understand that the Navy has no authority over aviators – or, rather, that the Corps will not let them have any authority. Imagine navy-men trying to arrest every dragon that threw a hissy-fit during wartime. It would be chaos. The aviators only pretend to obey foreign officers, on smaller matters, because otherwise the admirals might complain and the Corps could face considerable pressure.

In any case, Granby thinks he could have shot Barham and Admiral Lenton would have defended his decision, however unsuccessfully. He half-wishes he'd done it, except that Laurence would have fussed something horrible and probably taken responsibility for that too.

Granby watches as Laurence collapses onto his bed, tugging uselessly at his cravat before evidently deciding the effort is too much; he lays back with a faint grimace instead. Granby makes an aborted motion forward, but he feels Laurence would not appreciate an offer to help.

“Sir,” Granby says at last, because he must say something. “I know it is not what you'd like to hear, but I would not give a damn if they tossed me from the service for this. Admiral Barham was entirely in the wrong, and when the day comes that I must obey such a man then I will leave the Corps myself.”

Laurence looks very pale. It may be the blood-loss, or the man's injured head; perhaps he is just horrified at Granby's temerity.

Something in Granby softens. “And I think all the crew would say the same,” he adds, more gently. “But do not fret about us, I beg you. Remember you must meet with Lenton and Barham tomorrow,” though if it were Granby's decision Laurence would be resting the rest of the week at least.

He halts by the door a moment, but Laurence still says nothing. The poor man looks entirely disoriented. Perhaps he took some of that laudanum after all. Shaking his head, Granby leaves him to rest.

He spends the rest of the night with Temeraire, fielding queries from the crew who all want to know if they should spirit the captain away and damn the consequences. Granby actually considers it – a damn meeting, after an injury like this! – but ultimately he consoles the crew with platitudes and sends them away.

He cannot help but remember Laurence's bleak voice. “I cannot condone your actions,” he'd said, “...But I will try to plead for leniency on your behalf.”

Granby shakes his head. He will give Laurence no cause for additional worry; he will ensure that the crew are meek and proper as lambs, if only it will comfort the man.

Unless Barham manages to arrest Laurence after all. If that happens -

Well, how hard could it be, to stage a jail-break with the help of a dragon?

Effect of Wireless Sensor Networks to Increasing Causes of Numbness by Md Rahimullah Miah*

Abstract

Background: Numbness is a complex sensor disease in legs, feet, lower parts of the body or entire body. Yet Medical professionals are facing the insufferable supplementing causes of numbness in individual’s body as a very important global health issue since the 20th century.

Objective: The study aims to evaluate the applications of the fluctuated and processed radio frequencies that effect limbs or other organ of the body within and around the individual’s body boundary at GPS locations. Everyone uses mobile phone within GPS location, but none can know its impact.

Methods: This impact identifies through ISNAH Experiment on cat and dog through application of processed wireless sensor networks at open activities eyes and GPS positions.

Results: This study represents the numbness with sudden pain due to misuse of wireless sensor networks towards an individual’s body at light and dark environment. The research also focuses on the more effective enlarging causes of numbness in dark than light environment. These findings replicate the implication in numbness through operative treatment and recovery that the surgeons provide, which cannot improve efficiently due to abusing wireless sensor networks. The study also found the digitalized health systems are in risks to insecure advances sensor technology.

Conclusion: Systematic healthcare awareness is vital for management with modern technological device but such awareness is still below par, which is alarming to individual’s good health. The study suggests future research trajectories of a new sophisticated alternative secure treatment approach to promote healthcare in the priority of Sustainable Development Goals 2030.

Introduction

Nowadays numbness augments in legs or feet or lower part of the body in sudden among human beings [1-4]. Numbness of body is a silent sensor disease, which is numbed from the waist down to the legs [5,6]. Suddenly some male or females may feel numbness in their legs and feet or lower part of the body or entire body because of sitting / sleeping in a position that puts too much pressure on the nerves or reduces blood flow or lack of body electron movement [7-10]. However, continuing or unexplained numbness may be a mark of a causal medical condition [11-13]. Long-term numbness or a tingling feeling in the legs, feet, part of body or entire body may be because of conditions in several ways, such as multiple sclerosis, diabetes, peripheral artery disease, fibromyalgia or unwanted sensor networks [14-18]. A lack of pharmacological tools has delayed our understanding of the physiological mechanisms of causal tingling paresthesia. It may feel the sensation in the entire leg, below the knee, in different areas of foot or lower part of the body within a body boundary of the existing GPS locations [19-22]. From higher study research, the researcher finds new ideas of the reasons any person might experience numbness in the legs, feet or lower part of the body or in the full body suddenly numbness, along with identified symptoms, treatment with recovery systems [17,23-26]. The aim of the study is to find out the root causes of numbness through processed wireless sensor networks to solve with core challenges in worldwide public health security.

Methods and Materials

We conducted this research method at as PhD research work from October 2014 to October 2017 at the Universiti Malaysia Sarawak (UNIMAS), Malaysia. We connected the method with different parameters to enhance data collection, ISNAH Experiment, Specimens Tracking Process, Data Analysis and interpretation as below

Data Collection

All specimens housed in a room with controlled temperature 36.4°C in cat and 36.7°C in dog with breathing rates, respiration, blood pressure and feline body mass index.The experimental design randomly divided into three experimental groups with Body Mass Index: obese, normal and thin and observed the impact of wireless sensor networks towards pancreases among them in the light and dark environments. The study causes an integration of methods used in wireless sensor networks towards animals’ body and identified its implication. This envisaged the research taking in matter-of-fact research elements to investigate issue hoisted in the study, primarily targeted at SMART devices like telematics’ users towards specimens. Telematics is a smart device, comprises scanner, Global Positioning System (GPS) and Global Navigation Satellite System (GNSS). The fieldwork conducted in the studied area from January 2015 to January 2017.

ISNAH Experiment

Sensor technology comprises ISNAH Experiment. It implies the experiment on the Impact of Sensor Network on Animals and Human beings (ISNAH). The cyber trackers misuse the sensor technology to augment non-communicable diseases among animals and human body. The study examined into two specimens, one is dog and another one is cat among 14 individuals for identification of this misuse application. These animals are available in the study area and suitable for experiment. The study selected sound health two species with Feline Body Mass Index (FBMI) and other following parameters as shown in (Table 1).

The experiment took on dark and light conditions. The specimens stayed in specific geographic location and put the individual inside the iron case (size: 3.5′x 2′x2.5′). Then measurement of individual’s coordinates location includes longitude, latitude and ellipsoid height with GPS and GNSS identifiers.From the field observation, the Automated Radio Telemetry System is more effective in dark than light environment. For this purpose, the study was to examine the system with on smart cell phone, telematics device, iron cage and individual species separately. The dog and cat put into the iron cage with cell phones separately.

The experiment continued at five locations, viz.

Location A with light environments but no Wi-Fi,

Location B with dark and light environments including Wi-Fi,

Location C with dark and light environments including Wi-Fi,

Location D with light environments but no Wi-Fi,

Location E dark and light environments including Wi-Fi.

These experiments continued to identify the reflection of Automated Radio Telemetry System from Telematics device via cell phone towards animals at 09:00 p.m. to 6:00 a.m. from 1 January 2017 to 28 January 2017. The location of experiment settled the species with the temporal conditions through global positioning system. Although many important moments in animal’s life are difficult to study because they are rare, cryptic or occur over large spatial or temporal scales. For the study of FBMI calculation, we used the study web calculator through using rib case circumference and length of the lower back leg from the knee to the ankle.

Specimens Tracking Process

Sensor networks track on animals and human beings, where contains in blood circulation and movement of electron. The ISNAH experiment interlinked electron through tracking process. This process included several steps which enhanced to fulfill the Sensored observation. The study was observed the physical conditions including non-communicable diseases of animals like diabetes affected by the telematics device through misapplication radio frequency through tracking process as shown in (Figure 1). Different stages of Tracking Process of Radio Frequency towards animals are listed as below:

Selective communication devices,

Searching object and scanning of individuals body organ,

Identify body organ and light and dark environment,

Sensored the specimens with high, normal and low radio frequency,

Observed and compared the specimens status,

Feedback meeting and illustrated the consequences at result and discussion.

Data Analysis and Interpretation

Quantitative and qualitative related bio-sensor data got through field observation, interviews, field surveys and ISNA experiment while we got secondary data from diverse sources. All general information regarding the occurrence of specimens, status and affected condition checked for accuracy from the unique sources and sources of information also verified. We collected information regarding the initiatives of the authority towards the geographic locations through relevant secondary information and field survey. The compiled and processed data involved in preparing data master sheet and assimilated into suitable systems used in the results and other segments consecutively. The data compiled and analyzed for presentation and interpretation using standard data analysis software like MS Office Suite 2019 and SPSS version 27.

Results

Occurring of Numbness with Sensor Technology

Due to misuse of fluctuated radio frequencies within a GPS location, any animal or human beings suffers in numbness in actual time within specific radius of the individual’s body boundary. The processed frequencies create the digital poisoning within the circumference of an individual’s optical distance. Because of this, it blocked electron movement or blood circulation with poisoning in GPS location. If any person misuse the telematics or relevant sensor device towards he / she or both of them, they suffer in numbness at the poisoned location.

Suddenly or after a few moments at sitting or sleeping, a male or female’s legs go numb temporarily because of their posture or staying in an existing environment. This numbness creates as a chronic or long-lasting in a specific of the body, which is almost always a sign of an underlying medical condition or physical structure. Suddenly causes of numbness include

fluctuated radio frequency,

posture,

sleeping gaps,

injury,

sitting gap,

diabetes,

clinging gap,

lower back issues,

sciatica,

tarsal tunnel syndrome,

carpal tunnel syndrome,

peripheral artery disease,

tumors,

abnormal growths,

fibromyalgia,

multiple sclerosis,

stroke, and

alcohol use.

There are some specific symptoms for numb legs or feet or lower part of the body. Numbness is just one of the many symptoms associated with temporary and chronic numbness. Many people with numbness in their body organs experience additional symptoms at the same time or intermittingly including

tingling,

burning,

tickling,

itching,

crawling feeling under the skin, which as shown in (Figure 2) (Table 2).

If an individual suffers in numbness, he/she can take some simple steps to protect his/her limbs, reducing discomfort and improve mobility. Many people believe it is a crippling and practicable part of growing old. But effects are changing through the expansion of innovative technology. Treatments are better with innovative technology due to open-closed eyes systems, which illustrated in discussion part. Plenty of aged-people are well with little in numbness owing to sensor technological awareness.

The study mentioned that he/she is sitting or sleeping at home or in the office. If he has a mobile phone or sensor device around him with open eyes, his location knew easily. Even if he doesn’t have a mobile phone, we can know your location through laughing, crying, yawning, coughing, sneezing, talking, speaking, flattering, or being with open eyes. The distance between the various parts of individual’s body knew through the telematics device around him, and then digital scanning does with software like Computed Tomographic scan or Magnetic Resonance Imaging, the individual’s hand, legs or lower part of the body or whole body feel numb suddenly. Remote sensing device then applied the fluctuated-frequencies with electromagnetic force to the connective tissue in the area via telematics. After the few moments, fluctuated mobile sensor particles in the blood vessel contracts, the blood flow stops and the connective tissue becomes useless, then the leg or hand or lower part of the body becomes in numbness, which as shown in (Figure 3). In light environment, numbness occurs in 25 minutes and in a dark environment, it takes 12 minutes. However, optimum uses of radio frequency are suitable for body boundary. It is helpful to movement or straight of the body or part of the body. But suddenly fluctuated sensor radio frequency is harmful to all living beings. Discussion

Sensor technology is a blessing to all generations for sound health in a sound mind. But some cyber hackers are misusing the processed radio frequency with sensor technological devices and optical sights within GPS locations [27-30]. They create abnormal situation towards an individual’s body within the body boundary area. Then he/she affects in numbness either temporary or long-lasting [31-34]. Many people say their legs have “fallen asleep”, which is termed in medically as transient paresthesia [35-40]. Habits that can cause the body or part of body to fall asleep including

crossing the legs for too long,

sitting or kneeling for long periods,

sitting on the feet,

wearing pants, shoes, socks, gloves that are too tight [41-45].

Most people with fibromyalgia experience a variety of symptoms including

stiffness and soreness for no apparent reason, especially in the morning or after sleeping,

chronic exhaustion,

memory problems and difficulty thinking clearly, sometimes called fibro-fog,

restless leg syndrome [46-50].

Almost everyone with fibromyalgia suffers symptoms in over one part of their body for at least 3 months at a time. Any other symptoms do not accompany if numbness in the legs and feet or is not long term, it is unlikely to be caused by fibromyalgia. The tarsal tunnel is a narrow space on the inside of the ankle [51,52]. Male or female with tarsal tunnel syndrome felt numbness, burning, tingling, and shooting pain in their ankles, heel and feet. It linked this type of nerve damage to reduced levels of B vitamins, such as B-1 (thiamine), B-9 (folate), and B-12, which is caused by excessive alcohol intake [53-56]. The legs are one of the most common parts of the body affected by Peripheral Artery Disease (PAD) [57-59]. Most people with PAD experience pain and cramping in their entire body or part of the body, or in their legs and hips, when they are walking or going upstairs [60-65]. Some are with PAD also experiences leg numbness and weakness. Symptoms of PAD typically go away after a few minutes of rest [66-70]. However, the mentioned causes occurred by the missing of sensor technology, which as shown in (Figure 4). Though they may occur these as usual, either certain conditions. The numbness can cause problems correlated to sensing, feeling, and moving of the body or part of the body [53].

Some patients suffer from the symptoms as numbness in their hands or feet. After lying on one side of the bed at night for a while, the hands and feet on that side feel numb. Because of these reasons, it is difficult to sleep at night. Sometimes, if the patients hold something in their hands for some time, their hands feel numb. After a while they can’t hold on anymore. The doctor observed the patients’ history that they couldn’t even hold the mobile phone in their ears for long while talking on the mobile phones [48]. Many wonders why this could happen, which needs to find the root causes. Because of this problem, the blood circulation in hands and feet is less than normal [71]. Then the pain occurs due to fluctuated pressure on the cervical spine or neck and lumber spine, or the nerves in the waist [56]. The numbness also happens in some diseases including cervical spondylosis, carpal tunnel syndrome, lumber spondylosis, varicose veins, peripheral neuropathy, diabetic neuropathy and motor-neuron disease. Due to lack of vitamins or minerals, the patients suffer in numbness. But the numbness occurs suddenly, the study takes the instant time towards patients’ numbness seriously due to misuse of fluctuated radio frequency. Because of which the numbness later becomes severe, it becomes difficult to get rid of the disease. Therefore, if such symptoms occur, it is necessary to take the advice of a specialist without delay and take treatment for recovery after diagnosing the cause [72]. Occasionally the limbs become numb, which is a symptom of some sensor diseases through misapplications of sensor technology within GPS locations.

Treatment

The proper treatment for numb legs, feet or part of the body depends entirely on the cause.

Medication

Medical options are for long-term numbness in the legs, feet or other organs of the body:

Antidepressants: Some antidepressants, such as duloxetine and milnacipran, have approved to treat fibromyalgia.

Corticosteroids: Some corticosteroids can help reduce chronic inflammation and numbness associated with conditions such as MS.

Gabapentin and pregabalin: Medications that block or change nerve signaling may help reduce numbness associated with conditions such as fibromyalgia, MS, and diabetic neuropathy.

Home-based Remedies

Home remedies that may help to relieve uncomfortable numbness in the legs, feet or lower part of the body through following parameters [73-76], which as shown in (Table 3x).

Challenges

At present, developing countries have no reliable sensor data despite substantial investments in digital health systems due to lack of sensor security [49,50]. The crowdsourcing data about providers, facilities and health measures is likely to grow more individuals with wireless sensor networks in risks. Artificial health intelligence and internet of everything use for better care and diagnosis, but these are expensive innovative technologies.

Conclusion

In conclusion, the study identifies the numbness limbs or part of the body within GPS locations because of the fluctuated radio frequencies. Based on this research, human beings and animals are not secure due to misuse of processed frequencies within body boundary wireless sensor networks in the existing environment. However, the study has attempted to improve a complete scenario of the speeding up causes of numbness in hands, feet, lower part of the body or complete body due to disseminating the fluctuated, homogenous and processed wireless sensor particles. The findings of this study obviously show for sensor network security and dynamic health policy towards present and rationalized generations. Everyone stays at optical sight with body boundary and innovative technology, but none can aware of its security systems. So, policy-makers, health experts and sensor technologists should develop a dynamic, secure health system in the future.

Hyperthyroidism is typically caused by a benign tumor (adenoma) on the thyroid gland — a butterfly-shaped gland at the base of the throat — that causes overproduction of thyroid hormone. This hormone regulates metabolism, heart rate and digestive function. When the gland overproduces thyroid hormone, the result is hyperthyroidism. Excessive production of thyroid hormone can cause serious, even life-threatening symptoms in kitties.

Symptoms to Watch for, Especially in Cats Over 10

About half of all kitties that develop hyperthyroidism have an increased appetite, but ultimately, about 90 percent of them lose weight because a side effect of excessive thyroid hormone levels is an increase in metabolism. Other symptoms of hyperthyroidism include:

Increased body temperature, heart and respiration rates (due to the up regulation of metabolic processes)

In a 2015 study, researchers analyzed the blood from 60 pet cats for the presence of flame-retardant chemicals. The objective of the study was to evaluate the differences in the levels of chemicals in healthy cats versus cats diagnosed with hyperthyroidism. Of the 60 cats in the study, 23 had normal thyroid function and 37 were hyperthyroid.

The study results showed that the hyperthyroid cats had higher blood levels of PBDEs on a fat weight basis. Another earlier study suggested that flame-retardant chemicals in house dust are linked to thyroid disease in cats. The study authors concluded that cats are primarily exposed to flame-retardant chemicals by ingesting house dust — which occurs when they groom themselves.2

Housecats seem to have extraordinary exposure to PBDEs. In 2012, Swedish researchers demonstrated that serum PBDE levels in Swedish cats were about 50 times higher than in the Swedish human population,3 and a 2007 study showed that PBDE levels in U.S. cats were 20- to 100-fold greater than median levels in U.S. adults.4

A more recent study sheds even more light on the connection between flame-retardant compounds and feline hyperthyroidism, suggesting that fish-flavored cat food could be a culprit.5 Scientists evaluated cat food and feline blood samples and discovered that the type of polychlorinated biphenyl (PCB) and PBDE byproducts found in both the food and blood samples are derived from marine organisms.

The researchers were also able to simulate the way in which the bodies of cats convert the type of chemical present in the food into the type of chemical seen in the cats' blood samples.

Based on their results, the team concluded that the byproducts detected at high levels in cats' blood samples likely came from fish-flavored cat food and not exposure to PCBs or PBDEs. However, further work is needed to determine the link between the metabolites (byproducts) and hyperthyroidism.

Very Important: An Expert's Recommendations for Thyroid Testing in Cats

According to Dr. Jean Dodds, an expert on thyroid disease in animals, one of the problems in diagnosing pets with thyroid disease is the use of standard laboratory reference ranges. There's just one reference range for cats, no matter their age, which makes little sense since we know from scientific studies that age affects thyroid levels.

Veterinarians typically run a number of blood tests during wellness exams for their patients, one of which is a total T4 (thyroxine) test. However, the results of this thyroid function test can be very misleading, because total T4 is affected by non-thyroid-related illnesses, a wide variety of drugs and excessive iodine in the diet.

Some vets treat patients based on just the T4 value, when it may or may not be appropriate. Dr. Dodds feels that to accurately diagnose thyroid conditions, we should be running a complete thyroid antibody profile. And vets should explain to clients up front that while the test is more expensive than a T4, it will also tell us what we can rule in or out.

As part of a complete thyroid antibody profile, Dr. Dodds recommends total and free T4, and total and free T3. Many in the veterinary community believe it's useless to measure total and free T3, but she strongly disagrees. The T3 values are necessary because in the case of a sick animal who has low levels in all four measures, it's much more likely to be a non-thyroid-related illness. Total T3 and free T3 are the markers that indicate a non-thyroidal condition.

Dr. Dodds also includes a TSH (thyroid-stimulating hormone) test for certain cats, in particular older kitties who've been treated for hyperthyroidism, often with radioactive iodine therapy. These cats have a tendency to become hypothyroid (not enough circulating thyroid hormone) within about two to five months because the gland is no longer working.

Dr. Dodds believes the way to monitor hypothyroidism in these cats is with the TSH test because it's very helpful in regulating doses of added T4 and T3 hormones to bring kitties back to a state of metabolic balance, returning to their pre-hyperthyroid condition.

Dr. Dodds and other veterinary endocrinologists she has collaborated with have conclusively decided that it is TSH that should be monitored in cats who have become iatrogenically hypothyroid (meaning the condition was induced unintentionally by a diagnostic procedure or medical treatment).

The TSH test is also important for cats with chronic kidney disease (CKD), because CKD can affect T4 levels, lowering them into the upper limit of the normal range. TSH has proved to be the most predictive test of hyperthyroidism in cats with chronic kidney disease. Dr. Dodds also points out that suppressing the thyroid in cats with kidney failure can worsen renal (kidney) tissue profusion, so we must be very careful treating these kitties. Some actually do better being slightly hyperthyroid when they have renal disease.

How I Treat Hyperthyroidism in Kitties

If it's early enough in the development of the disease, I like to start with natural therapies that avoid many of the risks and side effects associated with more traditional approaches. It's important to catch the disease early, which is why I begin monitoring cats' thyroid levels at age 7. If there's a slow but consistent increase in thyroid levels, I begin natural thyroid balancing protocols immediately.

There are many natural remedies that can be beneficial in managing your cat's condition if it's caught early. These include homeopathic remedies, acupuncture, herbal remedies (Eastern, Western and Ayurvedic) and nutraceuticals.

My second choice when a natural approach is ineffective is thyroidectomy, which is the surgical removal of the benign tumor from your cat's thyroid gland. This operation should only be performed by a skilled soft tissue surgeon who has performed many of these procedures. Successfully done, thyroidectomy cures the patient, which is the ultimate positive outcome.

Radioactive iodine therapy is another option, but the expense often prevents people from choosing this treatment. An alternative is medical management of the disease, which means putting the kitty on a medication called methimazole, which inhibits thyroid hormone production so the amount of circulating hormone is reduced.

There can be lots of side effects of methimazole, including gastrointestinal (GI) upsets that cause vomiting; however, I've had some success avoiding GI problems by using a compounded transdermal methimazole ointment applied inside the cat's ear. Occasionally, a patient will develop an almost immediate allergic response to the drug in the form of an intense facial itch that comes on after the first pill is administered. This reaction means the drug cannot be continued and another treatment option is needed.

Other side effects, which are fortunately rare, include decreased platelets and increased liver enzymes. The downside to medical management is that cats must take methimazole for the rest of their lives, and blood levels must be routinely monitored.

And since hyperthyroidism is known to mask kidney problems in cats, it's important for veterinarians to monitor not only thyroid hormone levels but also kidney function in kitties taking methimazole. I always start holistic kidney support via herbal and nutritional supplementation when the diagnosis of feline hyperthyroidism is made. Lecture 1: The Photograph as a Document

Notes from today (27/09/2017) Lecture by Neil Matheson

WHAT IS A PHOTOGRAPH? MY OWN SUMMARY

A photograph is a representation of reality - to an extent. Whatever the reality, it is still being recorded as it is seen, staged or not, unless you edit the image in post production. However there are things we cannot re create with the use of a camera. We cannot replicate the sound, or the 3d aspect of objects. But, even if you use a prop, a stage, set up people or actors to pose a certain way, you are still recording information, how those people look and what the location is like. Even if the photograph is subjective (photographers goal or point of view or metaphor within the image being conveyed to the audience) the evidence of what is there in front of the camera is real.

HISTORY OF PHOTOGRAPHY

A photograph is a documentation of reality and most commonly used as a way of scientific documentation. Photographers like Anna Atkins (in 1843 created a series titled ‘British Algae’) would take physical plants, specimens, and place them onto a paper creating a realistic and accurate representation of whatever flower or plant it was. Scientists used photography as a method of documentation, from a very early stage. The process used was called Cyanotype, and old process. Another example would be ‘Dog Grave’ by Thomas Mailander.

Using the Cyanotype to create art is very interesting to me, I love the impressions made onto the paper as well as the colours of blue and the ‘scientific’ element to them.

The photograph was first invented as a dark room, which within stood an artist. one of the walls would have a small hole in the middle, and the idea was that whatever was outside would be projected through the dark room and onto the wall on the other side; a moving, live image of what was going on outside around them. Then the artist in the middle of the dark box would trace the outline of the projection.

Then this idea was simplified, from a room to a box. This time the image appears onto a mirror, and again is traced by an artist. So, the next step was to actually ‘capture’ and freeze the image still.

In 1844 the Daguerrotype was invented, the first image being of the photographer and creator himself, a self portrait in the form of a positive film. This began the idea of the image creating itself in a way, the machine creating the image ‘without human intervention’.

Soon after Daguerre came out with his technique, so did Henry F Talbot in the same year 1844. “The open door”. His process created a much softer image, using wax paper and producing a negative film. The process produced less detailed or precise images. One of his photos is titled “Articles of China”. Henry’s view on the process was that it was “The Pencil of Nature”. An idea of nature reproducing itself, again automatically without man’s intervention.

His image, articles of china, shows the practical uses of the photograph early in it’s making. Taking a photograph of his plates or china, shows what possessions he had, and this would be photographic evidence of that, and if they were stolen he would use the photograph as proof - and considering that photography was still being seen as a mechanical operation or scientific form of documentation, it would be socially accepted by a court of law as proof (if submitted).

TYPES OF IMAGES

Semiotics - the study of signs. “Marks with meaning”, a visual language of photography.

Index - an image which has a direct connection with something, e.g. pictures of smoke - as the saying goes “no smoke without fire” so we know fire is related, and is the direct connection to the picture of smoke. Or close up of a finger print, you know it’s a finger, on a hand, on a human etc.

But both are real, and the camera will only ever imitate that which is in front of it.

We accept photos that have a story, that have social acceptance. Since a photograph can be used in a court of law, sometimes it’s hard to dispute that a image is evidence. The lack of focus in the famous image of the loch ness monster makes the image seem more plausible as if it was set up it would be in focus and there would be more detail. The man who took the image was a surgeon, making the person sound intellectual and therefore trustworthy, and this man never changed the story to his grave - he swore it was real. So the story and the way the image was taken, the amateur style, the graininess of the photograph, makes it all seem a bit more realistic. However of course people still question this.

The company which funded this campaign was later fined for bending the truth - using models instead of real people who had benefited from the charity’s expenses. This was the beginning of documentary photography.

DOCUMENTARY PHOTOGRAPHY

As the photographic process was still a slow one, with a big bulky camera and expensive big prints, the slow camera process meant that sometimes things would have to be staged in order to be able to capture them. This meant the spontaneous type of documentary photography we are use to now was not very possible in this time.

John Thompson photographed “Survivors of the street floods in Lambeth” in the 1870′s. Documenting the people affected by floods in a staged photograph, he was still able to capture the reality of the people, what they looked like, what clothes they wore, what the location looked like.

Jacob Riis - “Home of an Italian Rag Picker”

Documenting poverty and migration Jacob Riis photographed people who were trying to make it in America. This series was aimed at a middle class audience to inform people of the undercover side of America, what it was really like for people who didn’t live the middle class life, to convince them to donate and help these people.

He used the invention of the flash, a dangerous process involving magnesium to almost entertain the middle-class, somewhat insulting the poverty stricken people, by using them to entertain the rich ones. He later wrote a book called “The other half” which shows how he did not see himself to be on the side of those people, although coming from a migrant background himself.

Lewis Hine - “Slav Immigrant” 1905 Ellis Island

Unlike Jacob, Hine was not a article writer, and did not come from that background. He was an artist, he documented the poverty in a more dignified way, showing empathy in his photographs of the poor - he was on the side of the people and did not consider himself different, unlike Jacob Riis. He would sneak into factories, using different identities to get in with his camera he would sneakily take photographs of the child workers employed there, to later use in a campaign to inform the middle class (aired with statistics) that this is what these companies were doing to children. It was necessary to use a different identity and hide his gear in order to obtain this documentation of the life of a child worker, life inside a factory. Using statistic and fats as well as the images combined to make them socially excepted as ‘facts’.

Hine became a photographer of high integrity because of this, unlike Jacob.

He ended up romanticizing the ‘gang’ culture, making them seem just like normal kids, doing normal youth things. He didn’t portray them as a threat to society, like the middle class would describe them ‘gangs’, instead he portrayed them as positive members of society, a community of their own choosing.

The images are interior and exterior, sometimes focusing on squares and boxes (like the ones we put ourselves in) and he seemed to like walls as he would use them in his photos to show a metaphor of a very big authority, vs the tiny people. Or their big problems, vs. those unfortunate people who feel small or helpless. His photographs were empathetic also towards these people. Although they are composed well they still seem uncomfortable in a way.

I could hear things, and Icould feel terrible pain: when anaesthesia fails

The long read: Anaesthesia remains a mysterious and inexact science and thousands of patients still wake up on the operating table every year

When Rachel Benmayor was admitted to hospital, eight and a half months pregnant, in 1990, her blood pressure had been alarmingly high and her doctor had told her to stay in bed and get as much rest as possible before the baby came. But her blood pressure kept rising this condition, known as pre-eclampsia, is not uncommon but can lead to sometimes-fatal complications and the doctors decided to induce the birth. When her cervix failed to dilate properly after 17 hours of labour, they decided instead to deliver the child by caesarean section under general anaesthetic. Rachel remembers being wheeled into the operating theatre. She remembers the mask, the gas. But then, as the surgeon made the first incision, she woke up.

I remember going on to the operating table, she told me. I remember an injection in my arm, and I remember the gas going over, and Glenn, my partner, and Sue, my midwife, standing beside me. And then I blacked out. And then the first thing I can remember is being conscious, basically, of pain. And being conscious of a sound that was loud and then echoed away. A rhythmical sound, almost like a ticking, or a tapping. And pain. I remember feeling a most incredible pressure on my belly, as though a truck was driving back and forth, back and forth across it.

A few months after the operation, someone explained to Rachel that when you open up the abdominal cavity, the air rushing on to the unprotected internal organs gives rise to a feeling of great pressure. But in that moment, lying there in surgery, she still had no idea what was happening. She thought she had been in a car accident. All I knew was that I could hear things and that I could feel the most terrible pain. I didnt know where I was. I didnt know I was having an operation. I was just conscious of the pain.

Every day, specialist doctors known as anaesthetists (or, in the US, anesthesiologists) put hundreds of thousands of people into chemical comas to enable other doctors to enter and alter our insides. Then they bring us back again. But quite how this daily extinction happens and un-happens remains uncertain. Researchers know that a general anaesthetic acts on the central nervous system reacting with the slick membranes of the nerve cells in the brain to suspend responses such as sight, touch and awareness. But they still cant agree on just what it is that happens in those areas of the brain, or which of the things that happen matter the most, or why they sometimes happen differently with different anaesthetics, or even on the manner a sunset? an eclipse? in which the human brain segues from conscious to not.

Nor, as it turns out, can anaesthetists accurately measure what it is they do.

For as long as doctors have been sending people under, they have been trying to fathom exactly how deep they have sent them. In the early days, this meant relying on signals from the body; later, on calculations based on the concentration in the blood of the various gases used. Recent years have seen the development of brain monitors that translate the brains electrical activity into a numeric scale a de facto consciousness meter. For all that, doctors still have no way of knowing for sure how deeply an individual patient is anaesthetised or even if that person is unconscious at all.

Anaesthetists have at their disposal a regularly changing array of mind-altering drugs some inhalable, some injectable, some short-acting, some long, some narcotic, some hallucinogenic which act in different and often uncertain ways on different parts of the brain. Some such as ether, nitrous oxide (better known as laughing gas) and, more recently, ketamine moonlight as party drugs. (If you have an inclination to travel, take the ether you go beyond the furthest star, wrote the American philosopher-poet Henry David Thoreau after inhaling the drug for the fitting of his false teeth.) Different anaesthetists mix up different combinations. Each has a favourite recipe. There is no standard dose.

Todays anaesthetic cocktails have three main elements: hypnotics designed to render you unconscious and keep you that way; analgesics to control pain; and, in many cases, a muscle relaxant (neuromuscular blockade) that prevents you from moving on the operating table. Hypnotics such as ether, nitrous oxide and their modern pharmaceutical equivalents are powerful drugs and not very discriminating. In blotting out consciousness, they can suppress not only the senses, but also the cardiovascular system: heart rate, blood pressure the bodys engine. When you take your old dog on its last journey, your vet will use an overdose of hypnotics to put him down. Every time you have a general anaesthetic, you take a trip towards death and back. The more hypnotics your doctor puts in, the longer you take to recover, and the more likely it is that something will go wrong. The less your doctor puts in, the more likely that you will wake. It is a balancing act, and anaesthetists are very good at it. But it doesnt alter the fact that for as long as anaesthetists have been putting them to sleep, patients have been waking during surgery.

As Rachels caesarean proceeded, she became aware of voices, though not of what was being said. She realised that she was not breathing, and started trying to inhale. I was just trying desperately to breathe, to breathe in. I realised that if I didnt breathe soon, I was going to die, she told me.

She didnt know there was a machine breathing for her. In the end I realised that I couldnt breathe, and that I should just let happen what was going to happen, so I stopped fighting it. By now, however, she was in panic. I couldnt cope with the pain. It seemed to be going on and on and on, and I didnt know what it was. Then she started hearing the voices again. And this time she could understand them. I could hear them talking about things about people, what they did on the weekend, and then I could hear them saying, Oh look, here she is, here the baby is, and things like that, and I realised then that I was conscious during the operation. I tried to start letting them know at that point. I tried moving, and I realised that I was totally and completely paralysed.

The chances of this happening to you or me are remote and, with advances in monitoring equipment, considerably more remote than 25 years ago. Figures vary (sometimes wildly, depending in part on how they are gathered) but big American and European studies using structured post-operative interviews have shown that one to two patients in 1,000 report waking under anaesthesia. More, it seems, in China. More again in Spain. Twenty to forty thousand people are estimated to remember waking each year in the US alone. Of these, only a small proportion are likely to feel pain, let alone the sort of agonies described above. But the impact can be devastating.

For Rachel, sleepless and terrified in her hospital room, it was the beginning of years of nightmares, panic attacks and psychiatric therapy. Soon after she gave birth, her blood pressure soared. I was in a hell of a state, she told me.

For weeks after she returned home, she would have panic attacks during which she felt she couldnt breathe. Although she says the hospital acknowledged the mistake and the superintendent apologised to her, beyond that she does not recall getting any help from the institution no explanation or counselling or offer of compensation. It did not occur to her to ask.

Things can go wrong. Equipment can fail a faulty monitor, a leaking tube. Certain operations caesareans, heart and trauma surgery require relatively light anaesthetics, and there the risk is increased as much as tenfold. One study in the 1980s found that close to half of those interviewed after trauma surgery remembered parts of the operation, although these days, with better drugs and monitoring, the figure for high-risk surgery is generally estimated at closer to one in 100. Certain types of anaesthetics (those delivered into your bloodstream, rather than those you inhale) raise the risk if used alone. Certain types of people, too, are more likely to wake during surgery: women, fat people, redheads; drug abusers, particularly if they dont mention their history. Children wake far more often than adults, but dont seem to be as concerned about it (or perhaps are less likely to discuss it). Some people may simply have a genetic predisposition to awareness. Human error plays a part.

But even without all this, anaesthesia remains an inexact science. An amount that will put one robust young man out cold will leave another still chatting to surgeons. More than a decade ago, I found this quote in an introductory anaesthesia paper on a University of Sydney website: There is no way that we can be sure that a given patient is asleep, particularly once they are paralysed and cannot move.

Last time I searched, the paper had been adjusted slightly to acknowledge recent advances in brain monitoring, but the message remained the same: just because a person appears to be unconscious, it does not mean they are.

In a way, continued the original version of the paper, the art of anaesthesia is a sophisticated form of guesswork. It really is art more than science We try to give the right doses of the right drugs and hope the patient is unconscious.

The death rate from general anaesthesia has dropped in the past 30 years, from about one in 20,000 to one or two in 200,000; and the incidence of awareness from one or two cases per 100 to one or two per 1,000. Obviously we give anaesthetics and weve got very good control over it, a senior anaesthetist told me, but in real philosophical and physiological terms, we dont know how anaesthesia works.

It is perhaps the most brilliant and baffling gift of modern medicine: the disappearing act that enables doctors and dentists to carry out surgery and other procedures that would otherwise be impossibly, often fatally, painful.

The term anaesthesia was appropriated from the Greek by New England physician and poet Oliver Wendell Holmes in 1846, to describe the effect of the drug ether following its first successful public demonstration in surgery. Anaesthetise: to render insensible. These days there are other sorts of anaesthetics that can numb a tooth or a torso, simply (or unsimply) by switching off the nerves in the relevant part of the body. But the most widespread and intriguing application of this skill is what is now known as general anaesthesia.

In general anaesthesia, it is not the nerve endings that are switched off, but your brain or, at least, parts of it. These, it seems, include the connections that somehow enable the operation of our sense of self, or (loosely) consciousness, as well as the parts of the brain responsible for processing messages from the nerves telling us we are in pain: the neurological equivalent of shooting the messenger. Which is, of course, a good thing.

I am one of the hundreds of millions of humans alive today who have undergone a general anaesthetic. It is an experience now so common as to be mundane. Anaesthesia has become a remarkably safe endeavour: less an event than a short and unremarkable hiatus. The fact that this hiatus has been possible for fewer than two of the 2,000 or so centuries of human history; the fact that only since then have we been able to routinely undergo such violent bodily assaults and survive; the fact that anaesthetics themselves are potent and sometimes unpredictable drugs all this seems to have been largely forgotten. Anaesthesia has freed surgeons to saw like carpenters through the bony fortress of the ribs. It has made it possible for a doctor to hold in her hand a steadily beating heart. It is a powerful gift. But what exactly is it?

Part of the difficulty in talking about anaesthesia is that any discussion veers almost immediately on to the mystery of consciousness. And despite a renewed focus in recent decades, scientists cannot yet even agree on the terms of that debate, let alone settle it.

Is consciousness one state or many? Can it be wholly explained in terms of specific brain regions and processes, or is it something more? Is it even a mystery? Or just an unsolved puzzle? And in either case, can any single explanation account for a spectrum of experience that includes both sentience (what it feels like to experience sound, sensation, colour) and self-awareness (what it feels like to be me the subjective certainty of my own existence)? Anaesthetists point out that you dont have to know how an engine works to drive a car. But stray off the bitumen, and it is surprising how quickly pharmacology and neurology give way to philosophy: if a scalpel cuts into an unconscious body, can it still cause pain? And then ethics: if, under anaesthesia, you feel pain but forget it almost in the moment, does it matter?

Greg Deacon, a former head of the Australian Society of Anaesthetists, told me about a patient who was waiting to have open heart surgery. Deacon had been preparing to anaesthetise him, he said, when the man went into cardiac arrest. The team managed to restart the recalcitrant heart, then raced the patient into surgery, where they operated immediately. It was only once the operation had begun, the mans heart now beating steadily, that they could safely administer an anaesthetic. It all went well, said Deacon, and the man made an excellent recovery. Some days later, the patient told doctors he remembered the early parts of the procedure before he was given the drugs.

That is a sort of incidence of awareness which was thoroughly understandable and acceptable, Deacon told me: he had not even known if the mans brain was still working, let alone whether he would survive an anaesthetic. We were trying to keep him alive.

This is not denial. This is the tightrope that anaesthetists walk every day. They just tend not to talk about it.

In 2004, and against a backdrop of growing public and media concern, Americas Joint Commission on Accreditation of Healthcare Organizations finally issued an alert to more than 15,000 of the nations hospitals and healthcare providers. The commission, which evaluates healthcare providers, acknowledged that the experience of awareness in anaesthesia was under-recognised and under-treated, and called on all healthcare providers to start educating staff about the problem.

The American Society of Anesthesiologists subsequently acknowledged, in a 2006 practice advisory, that accidental intraoperative awareness, while rare, might be followed by significant psychological sequelae and affected patients may remain severely disabled for extended periods of time.

Before that acknowledgment was published, however, then ASA president Roger Litwiller made a small but telling observation. Despite his organisations concern about anaesthetic awareness, he did not want the issue to be blown out of proportion: I would also like to say that there is a potential for this subject of awareness to be sensationalised. We are concerned that patients become unduly frightened during what is already a very emotional time for them.

This is the anaesthetists dilemma. Under stress which affects just about everybody facing a general anaesthetic we lose our ability and often desire to process complex information. More than half of all patients worry about pain, paralysis and distress. High anxiety or resistance to the idea of anaesthesia may even contribute to anaesthetics failing, or at least increase the chances that we will remember parts of the operation. The more anxious we are, the more anaesthetic it may take to put us to sleep.

This creates a quandary for doctors: how much to tell? When we are anxious, our bodies increase production of adrenaline-type substances called catecholamines. These can react badly with some anaesthetic agents. So what does an anaesthetist tell a patient who, because of the type of operation, or their state of health, is at higher than average risk?

I mean, were trying to make people not worry about it, said one Australian anaesthetist I spoke with, but in the process I think we blur it so much that people hardly ever think about it, and thats probably not right either Should I be telling you that youve got a high risk of death? Is that going to frighten you to death?

Today the profession makes much of the emergence of a new generation of anaesthetists who are more attuned to the experiences of their patients. But the reality is that anaesthetists remain for the large part the invisible men and women of surgery. Many patients still dont meet them until just before or sometimes after the operation, and many, muffled in a fug of drugs, might not even remember these meetings. Nor do anaesthetists generally leave anything to show for their work: no scars or prognoses. When they do leave evidence, it is invariably unwelcome nausea, a raw throat, sometimes a tooth chipped as the breathing tube is inserted, sometimes a memory of the surgery. It is unsurprising, then, that by the time an anaesthetist makes it into the popular media, he or she is generally accompanied by a lawyer.

For the doctors who each day make possible the miraculous vanishing act at the heart of modern surgery, this invisibility can be galling. It is not surgeons who have enabled the proliferation of surgical operations numbering in the hundreds 170-odd years ago and the hundreds of millions today. It is anaesthetists. In hospital emergency rooms in Australia and other countries, it is not surgeons who decide which patient is most in need of and mostly likely to survive emergency surgery: anaesthetists increasingly oversee the pragmatic hierarchy of triage. And if you have an operation, although it is your surgeon who manages the moist, intricate mechanics of the matter, it is your anaesthetist who keeps you alive.

One of the first articles I came across when I started researching this subject was a 1998 paper by British psychologist Michael Wang entitled Inadequate Anaesthesia as a Cause of Psychopathology. Wang pointed out that pain even unexpectedly severe pain did not necessarily lead to trauma. Post-traumatic stress seldom followed childbirth, for example. What could be devastating, he said, was the totally unexpected experience of complete paralysis.

Even today, most patients undergoing major surgery have no idea that part of the anaesthetic mix will be a modern pharmaceutical version of curare, a poison derived from a South American plant, which causes paralysis. Few will be aware, either, that during surgery their eyes will be taped shut, that they may be tied down, and that they will have a plastic tube manoeuvred into their reluctant airway, past the soft palate and the vocal cords, overriding the gag reflex, and into the windpipe.

For the patient paralysed upon the table, said Wang, [t]he realisation of consciousness of which theatre staff are evidently oblivious, along with increasingly frenetic yet futile attempts to signal with various body parts, leads rapidly to the conclusion that something has gone seriously wrong. The patient might believe that the surgeon has accidentally severed the spinal cord, or that some unusual drug reaction has occurred, rendering her totally paralysed, not just during the surgery, but for the rest of her life.

As soon as anaesthetists explain to patients how the process works, it all starts to seem a lot less mysterious. And talk, it turns out, is not only cheap but effective: a preoperative visit from an anaesthetist has been shown to be better than a tranquilliser at keeping patients calm. I know from my own experience I had surgery on my spine how reassuring such a conversation can be. For me, it was not just the information; it was the fact of the human contact, of being treated as an equal, of being included, rather than feeling like an appendage to a process to which I was, after all, central.

Hank Bennett, an American psychologist, remembers a young girl whose mother brought her to see him some time after the girl had her adenoids removed. The surgeon referred the mother to Bennett after she had returned to him in a state of anxiety about her child. The surgery had been straightforward, but the mother felt that something was very wrong with her previously happy daughter: the child had withdrawn from her family and friends, and had stopped working at school. She could no longer fall asleep without her mother sitting with her, and was afraid of the dark.

Bennett spoke with the girl. He told her there must be a reason she had changed her behaviour, and asked if it might have something to do with the operation.

Bennett recalled: And she said, Yes. They saidthat they were going to put me to sleep, but the next thing I knew, I couldnt breathe. Now, she was only momentarily like that she does not remember the breathing tube going in but when I asked why she was doing these things differently at school and at home, she said: Well, I have to concentrate and I cant be bothered by anything. Ive got to make sure that I can breathe.

Bennett referred the girl to a child psychologist, and within weeks she was back to herself. Today she would be approaching middle age. But lets say that was just luck, Bennett says now. What if nothing had been picked up about that? Would she have been permanently changed? I think that you would say, yes, she probably would have been.

So if you were my anaesthetist and I your patient, there are some other things Id hope you would do in the operating theatre. Things that many already do. Be kind. Talk to me. Just a bit of information and reassurance. Use my name. Patients who remember waking are often greatly relieved at having been told what was happening to them, and reassured that this was OK and that they would now drift back to sleep.

The Fifth National Audit Project on accidental awareness during general anaesthesia states: The patients interpretation of what is happening at the time of the awareness seems central to its later impact; explanation and reassurance during suspected accidental awareness during general anaesthesia or at the time of report seems beneficial. Hospital staff could put a sign on the wall of the operating theatre: The patient can hear. Because one of the strange things about anaesthetic drugs is that they can exert their effect in each direction not just upon the patient, but upon the doctors and theatre staff performing the procedure.

After the teenage son of a good friend was badly burned in an accident some years ago, he had to endure weeks of intense pain, culminating each week in the agonising ritual of nurses changing the dressings on his chest and arms. They did this by giving him a dose of a sedative drug designed to distract him from the pain and prevent him remembering it. My friend would attempt to comfort her son as he yelled and as the nurses got on with their difficult task. What she observed was that while the drugs did give her son some distance from his pain, and certainly his memories of it, they also gave the nurses some distance from her son. It was an understandable, perhaps necessary, distance; but inherent in that tiny retreat (the lack of eye contact, the too-bright voices) was a loosening of the tiny filaments that connect us one to another, and through which we know we are connected.

It is a process inevitably magnified in the operating theatre, where the patient is silent and still, to all intents absent, and where their descent into unconsciousness is routinely accompanied by the sounds of the music being cranked up (one prominent Australian surgeon is said to favour heavy metal), and conversation. It need not take a scientific study to tell us that this deepening of respect and focus is good not only for patients, but for doctors, too. In the end, it might not even much matter what you say. During an operation, a soothing voice may be more important than what the voice says, writes psychologist John Kihlstrom, who still encourages anaesthetists to talk to their anaesthetised patients (about what is going on, giving reassurance, things like that) but acknowledges that he doesnt expect them to understand any of it not verbally at least.

Japanese anaesthetist Jiro Kurata calls this care of the soul. In an unusual and rather lovely paper delivered at the Ninth International Symposium on Memory and Awareness in Anaesthesia in 2015, he wondered if there might be part of our existence that cannot ever be shut down, which we cannot even conceive by ourselves a subconscious self that might be resistant to even high doses of anaesthetics. He called this the hard problem of anaesthesia awareness. I have no idea what his colleagues made of it. But his conclusion seems unassailable.

Any solution? Science? Yes and no. Monitoring? Yes and no. Respect? Yes. We must not only be aware of the inherent limitation of science and technology but, most importantly, also of the inherent dignity of each personal self.
