ok well if helsknight has top surgery scars that begs the question: did he spawn in with top surgery scars or is there a top surgeon somewhere in hels? ordinarily id assume it'd be zedaphs hels but.... you know...

There is definitely gender affirming surgery in hels. An entire city full of people trying to find identity after being brought into the world by someone who claims to be their complete opposite? Gender affirming surgery and hormones were probably made right after city infrastructure and food carts.
