Oh my goodness, this is a hard quesiton but from the top of my head, my favorite books are The Curious Incident of the Dog in the Night-Time by  Mark Haddon & Before Ever After by Samantha Sotto. :3 There’s this FIlipino author named Lualhati Bautista whose writing style is just amazing & she’s a badass who’s not afraid to speak her mind even if it’s against those in power

Both my parents are lawyers & my siblings are about to take the bar exam so there’s a lot of pressure for me to follow their footsteps but I’m pretty proud that I haven’t given in to that (even if my mom occasionally drops the are you sure you’re not going to law school? question in a way that makes it obvious she wants me to)

I’m part of a family of 5 and I’m the youngest buuut I have a lot of cousins because my grandmother has 11 siblings. As ironic as it sounds, I’m closer to those who moved to the US than those who are still here in the Philippines.

Indicações de Zysertin

ZYSERTIN é indicado para o tratamento e prevenção de recidivas1 de sintomas2 de depressão, com ou sem ansiedade ou história de mania; transtorno obsessivo compulsivo (TOC); transtorno do pânico, com ou sem agorafobia3; transtorno obsessivo compulsivo em pacientes pediátricos; transtorno do estresse pós-traumático (TEPT); síndrome4 da tensão pré-menstrual (STPM) e/ou transtorno disfórico pré-menstrual (TDPM) e fobia5 social (transtorno da ansiedade social).

Contraindicações de Zysertin

ZYSERTIN é contra-indicado em pacientes que apresentam hipersensibilidade a qualquer um de seus componentes e em uso concomitante de inibidores da monoaminoxidase6 (IMAO7) ou de pimozida.

Precauções e Advertências de Zysertin

ZYSERTIN somente deverá ser utilizado na gravidez8 e lactação9, quando os benefícios superarem os riscos potenciais (categoria de risco na gravidez8: B). Recomenda-se que mulheres em idade fértil empreguem métodos contraceptivos, quando em tratamento. Há relatos de reações graves e, algumas, fatais, com o uso de sertralina associada a IMAOs, incluindo a selegilina e moclobemida. ZYSERTIN não deve ser usado associado ou nos primeiros 14 dias imediatos à descontinuação do tratamento com um IMAO7. Da mesma forma, após o tratamento com sertralina, deve-se aguardar 14 dias para iniciar o uso de um IMAO7. Também deve ser evitado o uso concomitante com outros fármacos que aumentem os efeitos da neurotransmissão serotoninérgica, como triptofano, fenfluramina ou agonistas 5-HT. Recomenda-se estrito acompanhamento médico, quando um ISRS for substituído por outro, principalmente quando estiverem sendo usados ISRSs de meia-vida longa, como a fluoxetina. Foram observados casos de mania / hipomania e convulsões, durante o uso de sertralina: recomenda-se controle rigoroso e a sertralina deverá ser descontinuada, em pacientes que desenvolvam convulsões. Pacientes susceptíveis devem ser cuidadosamente supervisionados, quanto à possibilidade de uma tentativa de suicídio, principalmente durante o período inicial do tratamento. Pacientes com insuficiência hepática10 devem receber doses menores ou menos frequentes de sertralina. Entretanto, estas não necessitam ser ajustadas, na insuficiência renal11. A segurança e eficácia da sertralina foram estabelecidas em crianças (de 6 a 17 anos de idade) somente para o tratamento do TOC. A posologia recomendada para pacientes12 mais jovens pode ser utilizada em pacientes idosos. Durante o tratamento, o paciente não deve dirigir veículos ou operar máquinas, pois sua habilidade e atenção podem estar prejudicadas.

Interações Medicamentosas de Zysertin

É contra-indicado o uso de sertralina associada a IMAOs. Foi demonstrado aumento dos níveis de pimozida, quando associada à sertralina. Não se recomenda o uso de ZYSERTIN concomitante ao álcool. Na co-administração de sertralina e lítio foi observado um aumento do tremor, sugerindo uma interação farmacodinâmica. Em pacientes que utilizam sertralina concomitante à fenitoína, é recomendado que as concentrações plasmáticas da fenitoína sejam monitoradas e suas doses, ajustadas. Entretanto, pode ser observada, também, redução dos níveis plasmáticos de ZYSERTIN. Se o uso concomitante de sumatriptano e sertralina for justificado, recomenda-se que os pacientes sejam acompanhados adequadamente. A co-administração de sertralina e varfarina promoveu um aumento pequeno, porém significativo, no tempo de protrombina13. Recomenda-se monitorar este parâmetro, quando for iniciado ou interrompido o tratamento com ZYSERTIN. A co-administração setralina e diazepam ou tolbutamida resultou em pequenas alterações, porém significantes, em alguns parâmetros farmacocinéticos. A co-administração com a cimetidina reduziu o clearance da sertralina. Não foram observados quaisquer efeitos ou interações com o atenolol, glibenclamida e digoxina. Não existem estudos clínicos estabelecendo os riscos ou benefícios do uso combinado de sertralina e terapia eletroconvulsica.

Reações Adversas ao Medicamento de Zysertin

As reações adversas mais frequentes foram: boca14 seca; sudorese15; tontura16; tremor; diarréia17, dispepsia18; náuseas19; anorexia20, insônia; sonolência; disfunção sexual (principalmente, retardo na ejaculação21). Foram, também, relatados alguns casos de midríase22; priapismo23; reações alérgicas; reações anfilactóides; astenia24; fadiga25; febre26; rubor; mal-estar; diminuição ou aumento do peso; dor abdominal e/ou torácica; edema27 periférico; hipertensão28; palpitações29; edema27 periorbital; síncope30; taquicardia31; coma32; convulsões; cefaléia33; enxaqueca34; distúrbios motores, como síndrome4 extrapiramidal, contraturas musculares involuntárias, parestesia35 e hipoestesia36; síndrome serotoninérgica37; galactorréia38; ginecomastia39; hiperprolactinemia; hipotireoidismo40; síndrome4 da secreção inadequada do hormônio41 antidiurético (ADH); aumento do apetite; constipação42; pancreatite43; vômitos44; tinido; leucopenia45; púrpura46; trombocitopenia47; distúrbios hemorrágicos48; hepatite49; icterícia50; elevação assintomática das transaminases hepáticas51; hiponatremia52; hipercolesterolemia53; artralgia54; agitação; reações agressivas; ansiedade; depressão; euforia; alucinações55; diminuição da libido56; paroníria; psicose57; broncoespasmo58; alopecia59; angiedema; fotossensibilidade cutânea60; prurido61; rash62 (incluindo raros casos de síndrome de Stevens-Johnson63 e necrólise epidérmica); urticária64; incontinência65 e retenção urinárias; visão66 anormal e sintomas2 de descontinuação da sertralina (agitação, ansiedade, tonteira, cefaléia33, náuseas19 e parestesias67).

Posologia de Zysertin

ADULTOS: A dose máxima recomendada de ZYSERTIN é de 200 mg/dia. Tratamento inicial: DEPRESSÃO E TOC - 50 mg/dia. TRANSTORNO DO PÂNICO, TRANSTORNO DO ESTRESSE PÓS-TRAUMÁTICO (TEPT) E FOBIA5 SOCIAL - 25 mg/dia e aumento para 50 mg/dia, após uma semana. Se não houver resposta ao tratamento com 50 mg/dia, as doses poderão ser aumentadas em 50 mg a cada semana, até a dose máxima recomendada de 200 mg/dia. As alterações das doses não devem ser realizadas com intervalos inferiores a 1 semana, devido à meia-vida de eliminação da sertralina, que é de 24h. Em geral, os efeitos terapêuticos têm início em 7 dias. SÍNDROME4 DA TENSÃO PRÉ-MENSTRUAL (STPM) E TRANSTORNO DISFÓRICO PRÉ-MENSTRUAL (TDPM) - 50 mg/dia, podendo ser mantido durante todo o ciclo menstrual ou somente durante a fase lútea, a critério médico. Se não houver resposta ao tratamento com 50 mg/dia, as doses poderão ser aumentadas em 50 mg a cada ciclo menstrual, até um máximo de 150 mg/dia, quando ZYSERTIN for utilizado durante todo o ciclo, ou até 100 mg/dia, quando utilizado somente durante a fase lútea. Se o tratamento for restrito à fase lútea, este deve ser iniciado com 50 mg nos três primeiros dias. Manutenção: A terapia de manutenção prolongada deverá ser realizada com a menor dose eficaz, devendo ser reajustada, conforme a resposta.
