A/N: HEY! This is where I start to change thinga up from the original. Riley is no longer her brother. I’m now going to be going with how I wanted the story to go originally.

Wade finally stopped laughing and you looked at him. “Are you done?” You whispered.
“Yes.”
“Do you have my gun?” You asked looking at his back.
“Oh. Yeah!” He pulled it from his back and tossed it to you which Steve caught. “I thought you would need this.”
“I don’t think she’ll be fighting anytime soon.” Steve shot back at Wade.
You turned and looked up at him. “What?”
“You can’t even stand. You have lost a lot of blood.”
You wanted to reply to Steve and tell him to shut up but the loud steps of the Hulk coming back so you maneuvered yourself to sit up. The hulk dropped the scientist in front of Steve and yourself.
“Thank you, Banner,” Steve said tossing some clothes to him. “How about you go change so you can look at Y/N’s wounds.” The Hulk nodded and walked into another room. Soon Bruce walked out and came straight to you and started to check you over as Steve stood up and grabbed the scientist’s collar and threw him against the wall. “Let’s have a chat.”
“So what hurts?” Bruce asked pretending Captain America wasn’t using a Hydra Scientist as a punching bag.
“Uh… my leg, where they cut me. I can’t see out of one eye.”
Bruce lifted your face. “Yeah… I won’t be able to see how bad the eye is for sure until we get back. You did good burning the cut on your leg though. It’ll be easier to fix.”
Steve socked the scientist in the face, “What was the plan here?”
“Why do you want to know?” He asked Steve.
Steve punched him again. “Because my wife is over there on the ground with one eye injured-”
“Actually it’s a little beat up but the eye is gone.”
Steve snapped and threw the scientist on the ground and cocked your gun back ready to fire.
“Whoa, Steve!” You tried to yell but it just made your throat hurt. You quickly signed to Clint.
“She says that killing isn’t you.”
“I think I can break my code for this piece of trash.”
Clint looked back at you so you could sign again. “She says that we should take him in and let Coulson deal with him.”
Steve looked at you. “Why the hell would we do that?”
You signed to Clint but Steve cut you off. “No, tell me!”
“He has a super-soldier serum.” You said in a very soft tone. “We need to back up all his research and then destroy it.”
Clint looked at Steve then at you. “I’ll get started on that then. Sam lock this psycho up and let Bruce and Steve deal with Y/N. Wade, go home.”
“Sorry, Katniss but I think I’ll stay with Y/N.”
“Wade. You should go home.” You said to him.
“But…”
“I’ll have someone contact you with an update later.”
Wade sighed. “Alright.” And with that, he got up and left.
Steve picked you up and cradled you. “We can talk later.” You whispered to him as he walked to the jet. “For now. I think I’ll sleep.”
“You deserve a good rest.” The said looking down at you. “You know I love you?”
“You’ve said it once or twice.” Steve chuckled and kissed the top of your head.

You woke up slowly thinking you were hearing your alarm. You reached your arm out only to have it land on someone's face.
You opened your eye a bit with a very confused Steve looking at you with your hand still on his face. “You okay?” He asked.
“Can you turn my alarm off?”
“It’s not an alarm. You’re in the hospital.”
“Ew… really?” You put your hand up to your left eye.
“They couldn’t save it.” He crawled into the hospital bed with you and held you close to him.
“I figured. Will I have a cool scar though?”
“Probably. And an Eye Patch.”
“Oo. I’m giving Fury a run for his money.” That earned a smile from Steve who leaned forward and kissed your patched up Eye.
The door slid open and the first thing you saw was a giant bouquet of flowers. “Damn mind if I join?” Bucky asked setting the flowers on a table.
“Why are you here?” Steve asked.
“I came to visit my savior. Calm down Punk.” Bucky came to your side of the bed and took your hand. “I really and honestly wanted to say thank you. I wouldn’t be here if it wasn’t for you. You saved my life. So I owe you. Big.”
“Don’t mention it.” You squeezed his hand with all your strength so he’d understand what exactly not to mention.
“Ow! Yeah. But I still owe you so. Look forward to it.” Bucky looked up at Steve. “Clearly I’m interrupting so I’ll leave.” He smirked at Steve and left the room after kissing you on the forehead.
You turned and looked at your husband. “You okay?” You asked him.
“Huh, Yeah. I just got a little beat up. Not as bad as you though.”
“Well, I am laying here missing an eye.”
“Speaking of... I know you won’t like it... But I’m going to take you off of fieldwork.”
You stared at him silently.
“Coulson agrees as well. Especially if you aren’t sure if they put the serum in you or not.”
“What?”
“I mean, it might not be so bad. You only started going into the field in Sokovia.”
“Which was multiple years ago..”
“Please. Y/N.”
“No. ‘Please Y/N.’ Steve, I’m one of the strongest people on the team.”
“You told me you don’t remember what they did to you.”
“They only experimented on me maybe a couple of times.”
“They still could’ve done something to you.”
The door opened again and Bruce walked in with his lab coat on. He peaked up from his glasses and the notes he had on his clipboard. “Steve, why are you on the bed?”
“He’s getting off now.” You said in a flat tone.
“Ah. You told her that she was taken off of the field.”
You gave Steve an angry look. “I’ve already been kicked off?”
“It’s not like you’d be going on a mission anytime soon,” Steve said gesturing to your leg.
“My leg is fine.”
“Actually you might need therapy.” You shot your brother a dirty look as soon as he opened his mouth.
“Only for a week.”
You just sat there silent as Bruce continued talking. But you didn’t hear any of it. How could they just kick you off! Your own husband! Now that you thought about…. You knew someone who wasn’t an Avenger who would let you fight. You gave a tiny smirk then made sure no one saw it. “May I at least go home?”
Bruce looked back and forth from you and Steve. “Yeah.”
“Cool. Have Sam or someone drive me. Steve has a slumber party with Bucky.”
You sat back and crossed your arms. Steve looked at you sadly but left knowing you needed to cool down. Bruce stayed and looked at you. “You’ll talk to him right?”
“Yeah, once I cool down.”
“You already regretting it?”
“A bit. Can I be alone for a little? I think I’m gonna sleep.”
“Okay. I love you, sis.”
“Love you too bro.”
Bruce left the room and you grabbed your phone and dialed a number. “Wade?”
“HEY! How you feeling?”
“They took me off fieldwork.”
“Shit. Wanna go shoot some people?”

“What is it that you are calling me out in the middle of the night.” You groaned as Bruce pulled you into his lab.
“Something happened to me!” He sounded panicked.
“What happened?” You looked around and saw the entire lab was destroyed. “Did something explode?”
“Sorta.” He started taking his lab coat off then and I realized he had no shirt on.
“Are you hurt?” You touched his arm and noticed he had a fever. “Do you want to go to the hospital?”
“No. They can’t help me.”
“Bruce! Just tell me. You’re scaring me.”
“No…” Bruce’s voice sounded different as he fell forward. “But I’m about to scare you.”
“Bruce?” You took a step forward but was thrown back against a wall.
You hit your head but didn’t blackout. You looked up slowly and there he was… A giant Green Monster.
He moved around fast. “Bruce.” You called out scared making him stop and spin around to look at you.
He jumped at you making you scream but he didn’t attack. You could feel his heavy hot breath blowing on your face. You opened your eyes and looked up at his face inches from yours. “Oh, dear… What did you get into now?”

You sat back leaning against the wall in the alleyway. You gave a long sigh as Wade finally walked up. “About time.”
“How did you get them to let you walk out of the hospital?” He asked as he eyed your wrapped up eye.
“They don't know I'm gone… yet. I made the sprinkler system go off.” You wrung out some more water from your hair.
“Your husband may kill you. Or me.”
“Got anyone specific we are going after?” You rested your gun on your shoulder.
“Well I thought the place we are going might be helpful to you specifically but are you going to be okay?”
You groaned and rolled your eyes. “I'm just blind in one eye.”
“And you have a giant cut in your leg…”
“It’s numb at the moment so I should be okay.” He looked at you worried. “I promise. I'm alright. I will tell you once I start to feel pain.”
Wade nodded and called the cab driver he likes. We sat in the car awkwardly. “You know I'm married to an Avenger right?” He nodded. “Then why are we taking a damn taxi? No offense.”
“None taken.” The driver smiles innocently. “Mr. Pool never pays me anyway.”
You gave him a dirty look. “Why don't you pay him?”
“The wallet makes lines in my suit.” Wade wined.
You rolled your eyes and took out a couple of hundreds from your wallet. “I hope this covers some of it. I'll pay back anything he owes you later.”
“Thank you, Mrs. Rogers. You are a very nice lady.” Wade gave him a dirty look. “So are you Mr. Pool.”
“Damn straight.” Your phone started reading, making you jump and Wade look at you confused. “You didn't even turn your phone off?”
“I didn't know I had it on me.” You looked at the number and suddenly you thought something bad had happened. “If you’re calling me someone has to be dead.”
“Haha… very funny.” Tony said with thick sarcasm. “I… heard what happened to you.”
“Seriously? From who? Steve…. Bruce?”
“The latter. I was shocked to hear from him.”
“So was I… Do you need something?” The taxi stopped and you and Wade started to get out.
“Where are you?” Tony asked when Wade slammed the Taxi door.
“Don’t avoid the question.”
“I just wanted to check on you…”
“Why?” You asked not fully believing him.
“I… was kind of an asshole last time we spoke.”
“Understatement.”
“Yeah. I’ve been patching things up with Steve…”
You stopped walking, “Really?”
“Yeah. I’m terribly sorry about the whole Accords mess. Can I maybe make it up to you two sometime?”
Wade tapped on his Adventure Time watch for you to hurry up. “Uh.. Sure. Listen, Tony, I have to go. I'm being annoyingly rushed.” Wade flipped you off.
“By who?”
“Doesn’t matter. I’ll talk to ya later.” You hung up the phone.

“You okay man?” Rhodey asked Tony when he rushed into the kitchen to grab a beer.
“I’m prepping myself.”
“For?”
“I told Y/N I had made up with Steve.”
Rodes chuckled. “You lying piece of crap.”

Steve knocked on the door softly. “Hey, Y/N, Are you up? I got you a cupcake from that place you like.” He opened the door a bit and saw an empty bed. “Uh…” He placed the box down and walked back into the hallway.
A nurse that was working at the counter saw him and gave him a confused look. “You need help?”
“Was my wife discharged?” He asked.
“No. The fire alarm and sprinklers went off after they broke this morning. When I went to get her it seemed someone had already taken her out. She’s not in there anymore?” He shook his head no. The nurse sighed. “Let me put an alert out. Maybe she went to the cafeteria. She was complaining earlier about how slow we were to bring her food.”
Steve grinned embarrassed and scratched the back of his head. “Sorry about that. She’s not a fan of hospitals. She spent a lot of time in one before.”
The nurse put a search out for everyone to check for her. In the meantime Steve pulled out the phone and dialed Sam’s number. “What’s up?” He asked as he answered.
“She’s gone. I’m going to call Bruce. You and Bucky start looking to where she could be.” He hung up and headed for the exit.

“Achoo!” You sneezed into your sleeve.
“Where did that come from?” Wade asked as we walked up to an old warehouse.
“I’m not sure I don’t usually have allergies this time of year.”
“Anyway. Everything is in there.”
“You are positive it’s Hydra?”
“Duh..” You could hear the eyeroll in his voice.
“Okay. Let’s go.” The two of you snuck into the building through a broken window. Inside you watched scientists walk back and forth carrying boxes. “What do you think is in there?” You whispered.
Wade looked around and tapped your shoulder. “I think he’s in charge.” You looked around him to see someone sitting on a wooden chair reading a file. “He looks a bit young…”
“Oh no…” You mumbled.
“You know the guy?”
“Yeah… I know him.” You looked down, “We have to go, now.”
You turned around but when you heard the gun click you stopped and held your hands up.
“Don’t move!” A soldier yelled. “We have some agents over here.”
“Trust us. We aren’t agents.” Wade chuckled.
“Three.” You mumbled. “Two….” You and Wade eyed each other. “Now” You swung your hand back and threw a wall of fire up as Wade started firing.
“So much for no powers.” Wade jokes.
“Shut up. It’s just a cover. Let’s go.” You both made it out of the window and started to run away. You almost made it off the property but suddenly both you and Wade were unable to move.
“Now what?” Wade yelled.
“He’s controlling our blood.” You said as both of your bodies turned around.
“I thought that was you!” The guy smiled. “It’s been so long. I was shocked when I heard that idiot scientist got you. I almost came to save you.” He walked up and got inches from your face.
“Look what he did to your pretty eye.” You flinched and shut your eye as he put his hand softly on your face.
“How about you leave her alone asshole,” Wade yelled. The man pulled a pistol out and shot Wade in the face making him fall.
“Sadly that won’t kill him.”
“I know. But I wanted to talk to you. You never come to visit me.”
“I didn’t know you broke out of prison.”
The man chuckled. “When you got your friends out I used that as my cue to get out as well. It was a long swim.”
“Go fuck yourself.”
He suddenly slapped you so hard you fell back. “That’s not how you talk to the love of your life.” He stopped and listened around. “Hmm… Bruce is nearby.” He leaned down and kissed your forehead. “I’ll take my leave now. See you around Y/N.”

“What happened?” Wade asked.
“God, You're so fucking heavy!” You groaned still dragging him on your back.
“Where are we?”
“Upstate New York.”
“You walked that far? With me?”
“We weren’t that far.”
“Who was the kid?”
You looked away and dropped him on the ground. “Don’t tell Bruce you saw him.”
“What aren’t we telling Bruce?” You looked over your shoulder at the iron man suit.
“Steve sent you?”
“Bruce actually. You good?”
“Just a slapped cheek. Wade here got shot in the face. But it’s not like it’ll make him look better.” You smirked at Wade.
“Fuck you. Help me up.”
You stuck your hand out and pulled him up.
“We found a base. It might be up in flames now.”
Tony sighed. “Y/N… I thought you weren’t..”
“She didn’t start it. I did it by accident.” Wade lied as he raised his hand like a school child.
“Is that true Y/N?” Tony looked at me.
You nodded but stayed silent.
“Alright then. Everyone is at the facility. Let’s go.”

“Geeze Y/N.” Steve sighed as he hugged you. “You gave me a heart attack.”
“Sorry. I hate Hospitals.” You gave him a quick peck.
“What happened to your cheek?” He asked to examine it.
“Wade picked a fight with some people. I had to step in after he got shot in the face.”
“Is that why you guys are suited up?” Bruce asked, walking up and crossing his arms.
“We ran into Hydra. Before you say anything. I honestly didn’t know.”
“She’s right I told her I’d take her to get not hospital food but instead I took her there.” He looked at me and back at Bruce. “Wait a minute.” You shook your head no really fast. “Why was that kid so damn touchy touchy with you if you’re married to Steve Fucking Rogers?
“What kid?” Bruce asked.
“‘Touchy touchy?’” Steve asks at the same time.
“I saw…” You gave Wade a dirty look. “I saw Riley.” You turned to Bruce. “He broke out. And he’s working with Hydra.”
Everyone but Bruce and Tony looked confused.
“Who’s Riley?”Steve asked.
“Y/N’s past mistake.”

We’re about 3 months away until the long anticipated release of the one, the only, the extraordinary, Kingdom Hearts III! Almost 14 years of waiting with about a dozen side games and HD re-releases are finally building up to this grand moment for video game fans. And like most other fans out there, I needed a serious catch up to remember all the convoluted lore and relive all the good times and bad with our favorite Keyblade wielders and Disney team members.
Since I don’t have all of the games on hand, much less the time to play the ones I own, I went to the wonderful world of YouTube and over the course of two months binged through about 30 hours of content, right from the very beginning! It was so much fun to relive all the series highlights, finally watch the newer content in the HD re-releases, and pick up on the little details I missed the first time around. And I thought I’d share some of the wonderful thoughts going on in my head during my binge.
Without further ado, here are Eliza’s silly but honest taglines, asides, observations, and comments of the Kingdom Hearts series!
Kingdom Hearts
The OG most ambitious crossover
I’ve been in this world for three hours and CAN’T FIND THE NEXT CHECKPOINT.
I know we jab at Chain of Memories for its awful gameplay, but I legit-erally didn’t finish the Atlantica world cause I had no idea where the fuck to go next. Even for a 2002 game, the mechanics were a nightmare sometimes.
Sometimes having every other world set up like a labyrinth is too much work than it’s worth.
Seriously, you have to talk to Aerith a million times to get Curaga. How the fuck were you supposed to know that without the Internet or a strategy guide??
Came for the bizarre mix of Disney and Final Fantasy– stayed for the characters and mildly convoluted story.
Just gonna add this in with the other “the power of friendship” crap I love.
Kingdom Hearts: Chain of Memories
Worst. Fucking. Fighting. System. EVER.
You ever wonder how the board meeting went for this game?
“So it’s, like, 50% the same game again, but shitty.”

Kingdom Hearts II
The best one. You can’t change my mind.
One man’s convoluted revenge scheme at the cost of, like, 5 teenagers’ mental well-beings.
I’ve only known Roxas for 6 days, but if anything happens to him, I’ll kill everyone in this room and then myself.
So Ansem wasn’t really Ansem. You see, he was this scientist’s apprentice who– aaaaaand I’m lost.
If your fandom experience at this point didn’t include calling Xemnas “Mansex” or listening to Dr. Bombay’s “My Sitar” to cope with Demyx’s 2nd boss battle, you did it all wrong.
We all made fun of Demyx for needing a notecard for his mission, but my work desk is a cluster of reminders and sticky notes, so I can’t judge anymore.
The beginning of the longest, most painful wait for a sequel.
I’m sure Haley Joel Osment is a chill dude, but who the fuck allowed him to sing?
Kingdom Hearts: 358/2 Days

No, seriously, I’m sorry, Terra is a massive idiot. Master Xehanort doesn’t even have to try hard to manipulate Terra; he’s just that goddamn dense and does a lot of stupid shit on his own. These are facts.

The Brooklyn-based Herbivores just released one of the sweetest soul collections in the midst of the panic and frustration of a worldwide pandemic. Their self-titled EP is inspired by the current season and all the fun that's usually associated with summer. Songs about love and the heat of a summer day are nostalgic when social distancing and face masks have become the new normal. But the current anxiety felt around the country regarding economics, health and the upcoming election has made the healing power of music while everyone is shut indoors even more valuable.
Musicians, songwriters and producers Bel-Ami, Redddaz, Misha Savage and Will Sacks were independent artists working in New York City before they became the Herbivores. They were drawn together by their desire to make music with each other as much as their love of plants. In this interview they talk about how the band was formed, the EP and being artists during COVID-19.

How did you all come together to become the Herbivores?
Misha Savage: The Herbivores initially came together as myself and Will trying to find somebody to sing all of these weird productions that we were making at his house. I had been in a project with Red for a number of years previously and what happened is we had called Amir up kind of out of the blue. Will and I had met at a jam session probably two years before we founded the group. And we just called him out of the blue and said we had some random songs that and it just sort of ballooned from there we called Red in for another one of these little productions we just wound up writing these kinds of songs together.
Do you all eat plant-based diets?
Bel-Ami: Not everybody fully all of the time.
Where does the name Herbivores come from?
Misha Savage: When we first started writing, both Bel-Ami and Red were plant-based. That combined with the fact that Will is an urban farmer/crazy plant dad. Seriously, he has twelve fruit and veg plants on his fire escape.
I’ve heard the EP and you’ve made it a two-part release, where did the idea of music for the summer and then the upcoming Nightshades for the fall come from?
Will Sacks: Originally at the top of the year we were planning to do four seasonal EPs, which were going to be four tracks each so almost like one song for every month of the year. But when COVID hit we had to cancel our release tour. We were going to play a few colleges we kind of regrouped and we said how can we put out as much light as possible right now. And you’ll see in a few months when we release the Nightshades project It has a very different feeling and tone.
Was there a conscious decision to make love songs was that the theme?
Bel-Ami: I don’t know if that was a conscious decision. When we are inspired we just go me being like mainly like a writer of neo-soul music that’s one of the things that I touch on heavily. Red does as well and like Will alluded to this record being much more of a summer vibe like these are the happier of the songs these are the lighter of the love. You’ll see in the Nightshades it gets a little deeper.
What’s the story behind “Kind of Like A Crush?”
Misha Savage: So we were playing basketball one day and we had smoothies it was a healthy day. I went home and wrote the whole thing in a night on Will’s synthesizer and I thought about a dancer I had met on my travels overseas and I included kind of like a crush in the title. We passed it over to Bel-Ami and he had written the entire song based on the title of the project.
Bel-Ami: Often times when I receive music from everyone they have a title on it and maybe it’s the mood they were going for but I actually glean a lot from that and typically I like to stay within that theme if I’m inspired by the music just because it feels a little bit more authentic to their creation. I write love songs and I crush all the time so I was able to draw on a lot of my experiences and it just flowed. I remember I was actually on my way to out of town I don’t remember where I was walking to the Megabus I might have been going to Maryland and I wrote it on my way from my house by the time I got to the Megabus I was already done so maybe like an hour trip. And I had everything exactly how I wanted it and rarely do I get inspired in that way but there’s a song that’s on our Nightshades EP that actually is the first song that I wrote with Misha and Will inspired me in the very same way. So it’s good to have those moments and I’ve had lots of those moments working with these guys.

Misha Savage: I played the guitar on that one.

What was the process for “Like You?”
Bel-Ami: The composition on that song really got me and I’m a sucker for a good guitar riff and often times when I write I draw on current, past maybe wanna be future experiences and everything just came.
Will Sacks: I sent it to Bel-Ami as I often do and what I got back were these gorgeous vocals honestly I don’t know if I told Amir this but I cried a little bit when I heard the hook the first time it gave me chills I really love that song and that’s me playing guitar too. We went back to Amir and we were like we’re keeping these vocals like I know you just recorded these at home remote pre-COVID before it was even cool. There have been three or four songs where within an hour of me sending something or Amir sending something Amir is like I’m ready to record.
Now you’ve already kind of touched on something I want to ask which is of course, how has COVID-19 impacted your creative and work lives?
Redddaz: The thing about COVID-19 is that it took the touring out because we were looking forward to going to colleges and performing. But for the most part, we have already been utilizing our work style pre-COVID in terms of working remotely so in terms of that it was never really an issue it was just the fact of getting out there and performing and sharing music with the masses. But in terms of the production quality, we’re still able to produce.
Did you all perform together live before the release of the EP?
Misha Savage: Yes, the last place we performed was at a place called at a venue called Elsewhere and it’s kind of crazy looking back like a full house of 308 people and looking back on it feels like a fever dream cause it feels like it’s been three or four years. That was the week before everything.
Redddaz: It was the last time we saw our entire New York family.
Have you been creating more since everyone has had to quarantine?
Misha Savage: It’s gone different ways for each of us Will and I took a lot of time to finish mixing this record and finish mixing a lot of the stuff that we had. I know that both Red and Amir have been also mixing and working on their own stuff at home and I do want to say one thing in that finishing this EP a lot of us and this is something I have talked about with Amir through finishing this work we can be inspired to finish the rest of our canon.
Redddaz: Just to piggyback off of that COVID gave us all a second to take a deep breath and to finish the things we had been tinkering with.
Misha Savage: We were planning to release mid-April before everything but having an extra second to take a breath to re-listen to everything remix everything, get it mastered, and really make sure that everything in our house was clean. For me, the first part of quarantine was like reading hundreds of pages of Buddhist literature and kind of getting back to a place where I wasn’t succumbing to the hustle and bustle of New York City anymore trying to re-center. So finishing this project I think for all of us has been really cathartic.
Have you come up with an alternative plan to promote the record now that COVID has killed concerts until further notice?
Misha Savage: We have a multi-prong strategy that’s a hybrid of the digital and in-person media. Right now we’re talking about doing rooftop style shows on myself and Will’s building that we can live stream. The conventional way has been thrown out the window one thing I was looking to early on in the quarantine is to start coding to make some video game for us to market.

When I was a junior in college I went to school in Vienna. One of my friends was Martina Nicholson. She became an OB/GYN, and just retired last year. She has a colleague, Dr. Richard Loftus, who is a biohazard virology-trained hospitalist. In other words he is treating COVID-19 patients in the hospital. I've read a lot of these accounts and this is the most powerful one I've ever read. It makes you want to hand out masks to everyone who's not wearing one. I am going to print this and carry copies with me when I'm walking. I will hand this out to people who don't have masks. If anybody gives me a hard time for my post on masks, you will get a copy of Dr. Loftus' brilliant, raw, painful account of what it's like treating these patients. What really struck me is how sick young previously healthy patients are 3 – 4 months post release from the hospital. Please be careful. Do not, for one more second, doubt this is real.

From my friend Dr Rick Loftus, MD. yesterday (7/2/20), update about Covid-19:
I'm in a hotspot hospital in a hotspot region (Coachella Valley, Inland Empire, CA). We just converted the entire second floor of our hospital to COVID-19 care yesterday, July 1. We have 65 inpatients with COVID-19 in a hospital with 368 beds. It is the same at our other 2 hospitals in the Valley. We spent yesterday deciding the ethical way to divide up limited remdesivir (30 patients' worth) for the hospital patients. My 20 incoming interns for our IM resident were exposed to COVID 2 weeks ago during their computer chart training; apparently 100% of our computer trainers had COVID19. One intern tested positive 7 days later and I insisted we re-test them all again, as there are almost certainly other cases with minimal symptoms. I raided my household and took my entire supply of face shields to the hospital for the residents to wear on their first day, and I paid $1000 of my own money to equip all of my residents with medical-grade face shields. I require all residents to wear a surgical mask or N95 with face shield if they are within 6 feet of another human, patient or coworker.
Roughly 20% of our inpatients die. Only 30% of our ventilated patients survive. (We try to avoid ventilation at all costs. Some people insist on being full code and decompensate despite high flow with face mask, proning, dexamethasone, antibiotics, and a cocktail of famotidine, zinc, Vitamin D, Vitamin C, NAC, and melatonin--we throw everything we can at each case, so long as it won't hurt them.)
My administrative assistant, who sits adjacent to the interns, just went home with COVID symptoms. Her test is pending.
In the Southwest, we are experiencing catastrophic exponential growth. I have had multiple families--siblings, parent-child, spouses--admitted with COVID-19. I had a 31 year old come in satting 78% on room air; he had been sequestering himself in his bedroom for a week to avoid infecting his elderly parents, with whom he lived. His sister, the only person he saw outside his immediate household in the 10 days prior to onset of fever, cough, and dyspnea, had also had fevers but had tested "negative" at our other large hospital so he thought it was safe to visit her. (Sigh. The Quest PCR test is about 80% sensitive, we think--it had emergency approval to sensitivity data was not required. The Cepheid rapid COVID PCR test is 98.5% sensitive but is in short supply due to limited reagent availability.)
I'm glad some of you are sheltered from what unbridled COVID-19 looks like. It's a hell show. This is *July*. What do you think my hospital will look like in winter?...
This is real. Doctors in places with proper public health responses will see few cases in their hospitals--like UCSF--but let me tell you something: The laws of physics and biology don't change. If you're in an unaffected region, an introduction and poor governance and low use of physical distancing and masks will give you an exponential increase in no time flat (i.e. 2-4 weeks). That's pandemic math. And 20% of the population infected needs a hospital. You *will* run out of beds with an unbridled pandemic. There is almost ZERO pre-existing immunity to SARS-CoV-2. There may be some "priming" of T-cell responses due to exposure to other "benign" beta-coronaviruses, but we have no idea if that explains the 20-40% of people who seem to get minimal symptoms. Asymptomatic infected persons, however, can, and do, spread COVID to those who die from it.
By the way: I've seen scary looking CT scans of the lungs that look like terrible interstitial pneumonia in a patient who had ZERO symptoms and SaO2 94% on room air. She came in for palpitations and the intern overnight got a chest CT for cardiac reasons. We didn't know it was COVID until her test came back 36 hours later. So "asymptomatic" does NOT mean "no biological activity." The virus replicates furiously in people who feel fine. Kids can spread this as easily as grown ups, even if they feel okay.
Related: I've talked to two previously healthy patients ages 32 and 44 who are 3 and 4 months, respectively, post their acute COVID. They continue to have cough, nightsweats, fever, fatigue. How many survivors have "post-COVID syndrome"? We don't know. Less than 20% but we're not sure. I've asked my hospital to allow me to establish a post-COVID clinic to care for and study survivors. Both NIH and UW are planning similar efforts based on my dialogues with them.
Autopsies show anoxic brain injury in many patients who died of COVID, not to mention microthrombi throughout the lungs and megakaryocytes in massive infiltrations in their hearts and other organs. People get heart failure, lung fibrosis, and permanent kidney injury from COVID-19. This is a disease of the vascular systems, and it can affect any organ, with lungs and kidneys being especially at risk.
In early May, thanks to lockdown, our census of 55 came down to 10 COVID cases, and for a brief moment, I actually had hope that the worst nightmares I had about COVID, as a biohazard virology-trained hospitalist, would not come to pass. Then we re-opened, without test/trace/isolate systems anywhere close to adequate. Eight weeks ago my county decided to make masks "optional," despite 125 doctors begging them not to do that. Now we're worse than we were in April. And it's getting worse every day.
You wanna see if COVID is real? Come walk on my COVID ward with me. It's real. Hearing people talk about it as if it's an exaggeration is, well, rage-inducing, honestly. Denial is the most common reaction to a pandemic. Denial is how the US will wind up with 1.1 million deaths instead of 30,000. I saw AIDS denialists get killed by their belief that HIV "isn't real, it's a pharma conspiracy of the medical industrial complex." Yeah, right, if you say so. I watched patients with those beliefs die.
The hardest part about this is, every new case I treat exposes me. I have assiduous hot zone technique. But no technique is bulletproof. If you keep exposing me to case after case, eventually, the virus will get through my defenses. I'm a 50 year old hypertensive. I don't expect to do well if I get infected. For now, I keep going to work. I'm one of the few pushing forward on COVID clinical trials, basic science, public health messaging, and diagnostic studies at my hospital. I feel a responsibility to keep going. I wake up with nightmares every morning at 4am. But I'm going to keep going for now. I feel very alone a lot of the time. People are not taking this seriously, and it's costing lives. -R
