Diabetes drug metformin is being investigated over contamination fears | Daily Mail Online
A diabetes drug taken by millions of people could be contaminated with a chemical linked to cancer, health officials fear.
Some metformin supplies in Singapore have been recalled after authorities found they contained small amounts of a chemical called NDMA.
The UK's Medicines and Healthcare products Regulatory Agency (MHRA) is now considering whether similar action needs to be taken for UK products such as Glucophage.
And the US Food and Drug Administration (FDA), Health Canada and the European Medicines Agency are doing the same investigations on their own turf.
NDMA is a chemical byproduct from pesticides, chlorine and industrial processes and is classified as 'probably carcinogenic to humans'.
The announcement comes after huge quantities of heartburn and indigestion drugs, including Zantac and UK and US supermarket brands, were recalled because of the same fears.
Diabetes patients are urged to keep taking their medication as normal.

Three out of 46 versions of metformin were recalled in Singapore last week because there were unsafe amounts of NDMA in them.
'The levels of NDMA seen in the affected non-UK metformin medicines are very low and appear to be within or even below the range that people would normally be exposed from other sources, including food and water,' an MHRA statement said.
'Patients in the UK are advised to continue taking their metformin medicines as usual.
'The risks from not having adequate diabetes treatment far outweigh any possible effects of the low levels of NDMA seen in metformin medicines outside the UK.
WHAT IS NDMA?
NDMA is the acronym for a chemical called N-Nitrosodimethylamine.
It is created by various industrial processes including the production of rocket fuel, pesticide use and the use of chlorine, from which it has leached into water supplies.
NDMA can be toxic to some enzymes in the body and to DNA, which may lead to cancer-causing mutations.
It doesn't degrade, or break down, naturally in the environment or our bodies.
It is also common in low quantities in many foods such as cured or smoked meats, fish and beer as well as tobacco smoke.
Animal studies have shown the chemical to cause colorectal, kidney, stomach and kidney cancers at high exposures.
In humans, on the other hand, studies have only suggested large amounts of the chemical could raise the risk of cancer, not cause it directly.
The US Food and Drug Administration says 96 nanograms is an 'acceptable daily intake limit'.

'As these metformin medicines are also available in Europe and outside the EU, the MHRA is working closely with the European Medicines Agency (EMA) and other regulatory authorities to determine whether any further action is required and will continue to keep patients updated as more information becomes available.'
Metformin is an extremely common drug in the UK and was prescribed, in various forms, around 22.2million times in 2018 at a cost to the NHS of £108.5million.

December 6: UK's Medicines and Healthcare products Regulatory Agency and the European Medicines Agency said they were investigating metformin products.
All authorities urged diabetes patients not to stop taking their medication.
The products recalled in Singapore are called Glucient XR (500mg) and Meijumet prolonged-release (750mg and 1,000mg), the Straits Times reported.
The FDA in the US and Health Canada are also looking into whether products in their countries might be affected.
'The FDA is investigating whether metformin in the U.S. market contains NDMA, and whether it is above the acceptable daily intake limit of 96 nanograms,' said Janet Woodcock, director of the FDA's Center for Drug Evaluation and Research.
The agency will 'recommend recalls as appropriate if high levels of NDMA are found,' she added in the statement.
A family medicine professor at Georgetown University in Washington DC, Dr Ranit Mishori, said: 'If these reports are true, it could be a big blow to many millions of people on this drug.
'There are many other alternatives and other medication classes but many of them have side effects, are more expensive and some of the newest ones may not be covered by insurance.'
Emma Elvin, senior clinical advisor at Diabetes UK, said: 'Many people use metformin to treat their type 2 diabetes – it is vital in helping manage the condition and reduces their risk of long-term complications.
'We recommend UK patients to continue taking metformin as prescribed, pending MHRA’s decision whether further action is required. If you have any concerns, please speak to your healthcare team.'
The investigations have been announced just weeks after the latest installment of an NDMA contamination scandal affecting heartburn and indigestion drugs worldwide.
Brand-name heartburn remedy, Zantac, was found to contain the chemical in both UK and US supplies and was recalled around the globe.
It triggered a domino effect with other brands and supermarket own-brands in the UK all forced to recall their own products after NDMA was discovered in them, too.

October 10, 2017 Popular Acid Reflux Drugs Promote Chronic Liver Disease

If you frequently watch daytime television, you are probably convinced, based on the advertising blitz, that most of the population has some type of stomach disorder. Unfortunately, that notion may not be far off reality, as 10% of the general population take a proton pump inhibitor (PPI) drug (e.g., Prilosec, Nexium, and Prevacid) to block stomach acid secretions and relieve symptoms of frequent heartburn, acid reflux, and gastroesophageal reflux disease (GERD). Interestingly, that percentage can be as much as seven times higher for people with chronic liver disease.
Now, investigators at University of California San Diego (UCSD) School of Medicine have just published evidence that stomach (gastric) acid suppression in mice and humans alters specific gut bacteria in a way that promotes liver injury and progression of three types of chronic liver disease. Findings from this study were released today in Nature Communications.
"Our stomachs produce gastric acid to kill ingested microbes, and taking medication to suppress gastric acid secretion can change the composition of the gut microbiome," explained senior study investigator Bernd Schnabl, M.D., associate professor of gastroenterology at UC San Diego School of Medicine. "Since we found previously that the gut microbiome—the communities of bacteria and other microbes living there—can influence liver disease risk, we wondered what effect gastric acid suppression might have on the progression of chronic liver disease. We found that the absence of gastric acid promotes the growth of Enterococcus bacteria in the intestines and translocation to the liver, where they exacerbate inflammation and worsen chronic liver disease."
PPIs are among the most commonly prescribed medications in the world, particularly among people with chronic liver disease. They are also relatively inexpensive medications, retailing for approximately $7 for a recommended two-week course of generic, over-the-counter Prilosec (omeprazole). But the frequency of use adds up, as one recent study estimates Americans spend $11 billion on PPIs each year.
The UCSD team wanted to determine the effect of gastric acid suppression on the progression of chronic liver disease. The researchers looked at mouse models that mimic alcoholic liver disease, nonalcoholic fatty liver disease (NAFLD), and nonalcoholic steatohepatitis (NASH). In each, they blocked gastric acid production either by genetic engineering or with a PPI (omeprazole/Prilosec). Moreover, the investigators sequenced microbe-specific genes collected from the animals' stool to determine the gut microbiome makeup of each mouse type, with or without blocked gastric acid production.
Amazingly, the researchers found that mice with gastric acid suppression developed alterations in their gut microbiomes. Specifically, they had more Enterococcus species of bacteria—which promoted liver inflammation and liver injury. The study authors noted that these phenomena increase the progression of all three types of liver disease in the mice—alcohol-induced liver disease, NAFLD, and NASH.
To confirm it was the increased Enterococcus that exacerbated chronic liver disease, the researchers colonized mice with the common gut bacteria Enterococcus faecalis to mimic the overgrowth of intestinal enterococci they had observed following gastric acid suppression. They found that increased Enterococcus alone was sufficient to induce mild steatosis and increase alcohol-induced liver disease in mice. Furthermore, the investigators examined the link between PPI usage and alcoholic liver disease among people who abuse alcohol. They analyzed a cohort of 4830 patients with a diagnosis of chronic alcohol abuse—1024 (21%) were active PPI users, 745 (15%) were previous users, and 3061 (63%) had never used PPIs.
The researchers noted that PPI intake among these patients increased stool concentrations of Enterococcus. What's more, the 10-year risk of a diagnosis of alcoholic liver disease was 20.7% for active users of PPIs, 16.1% for previous users, and 12.4% for never users. In other words, the rate of liver disease in people who chronically abuse alcohol was 8.3% higher for those who actively use PPIs compared to those who never used the acid-blocking medications.
The researchers concluded that there is an association between PPI use among people who abuse alcohol and risk of liver disease. However, they can't yet rule out the possibility that there could be other unidentified factors that differ between patients that do and do not take PPIs, which might confound the relationship between PPI use and liver disease.
"Our findings indicate that the recent rise in use of gastric acid-suppressing medications might have contributed to the increased incidence of chronic liver disease," Dr. Schnabl noted. "Although obesity and alcohol use predispose a person to acid reflux requiring antacid medication, many patients with chronic liver disease take gastric acid suppressive medications without an appropriate indication. We believe clinicians should consider withholding medications that suppress gastric acid unless there is a strong medical indication."
While there are inexpensive and readily available alternatives to PPIs, even non-PPI-based antacids (e.g., Pepto-Bismol, Tums, or H2 blockers such as Tagamet and Zantac) still suppress gastric acid to a lesser degree. While these other types of antacids were not tested in this study, Dr. Schnabl noted that any medication that suppresses gastric acid effectively could cause changes in gut bacteria and thus potentially affect the progression of chronic liver disease. Alternatively, nonpharmacological methods for managing heartburn are an option for some patients, including losing weight and reducing intake of alcohol, caffeine, and fatty and spicy foods.

I’m in the hospital again! 2016 just had to throw another one at me. I was back home for only four days when I spiked another fever. I also have really bad ulcers in the back of my throat that are preventing me from eating and drinking. So that’s fun. I was admitted straight to the cancer center this time on Wednesday night. Because I was so dehydrated, I had to get stuck seven times before they got an iv in. Plus two big needle sticks for blood cultures. It was a hellish night. That was on the 28th. The night after, I got sick after eating a few saltine crackers and threw up several times in a row. The next day went relatively okay, as did the next. Other than being starving and feeling absolutely parched, I did okay. Sunday was a nightmare. I felt feverish all day, both hot and cold at the same time. And so, so sweaty. I had bad stomach cramps and diarrhea most of the day. In the morning, I jumped up to go to the bathroom and ended up throwing up on the floor. Then again in the toilet. So I just stumbled over to the door and begged for my mom and Sam to come help me. While I was rinsing my mouth out in the sink, I threw up again, out my nose and everything. So there went the small number of crackers I ate that morning. Later that day I spiked another fever and had to be put back on IV antibiotics. I pretty much just felt shitty all day long. So weak and exhausted. It was like I couldn’t breathe or move at all. Then that night I started getting bad acid reflux. I had been handling it fine for a while, so I eventually just went to bed. But around midnight I woke up and felt restless. I turn over to sleep on my side, and the next thing I know I’m grabbing my puke bucket and throwing up a bunch of stomach acid. I was shaking and trying not to let the smell get to me, but I could tell I wasn’t done. I sat there for a few minutes, wiping down my face while Sam was rubbing my back. I was waiting for my nurse to bring me more IV zofran when I threw up again, twice in a row. It was horrible, and it burned so bad. My nurse brought me some Zantac syrup, which helped. I eventually fell asleep. I woke up bright and early this morning for a blood draw and vitals, and I’ve been feeling relatively okay so far today. My throat started burning bad again though, and I finally figured out how to soothe the pain. I diluted my mouth wash and it helped so much. I also swished with some viscous lidocaine. It was a godsend. I took all my medicine and drank a decent bit of water and a little bit of a vegan nutrition shake too. So today is definitely much better. I’m on IV dilaudid right now which is making me very sleepy. Sam is at work and I miss him a lot. He’ll be here in 3 hours or so. I think I’m gonna take a nap.
