The devastating impact of coronavirus on our health system if left unchecked

As a doctor writing about coronavirus, I have been asked by some of my medical colleagues to explain the impact that coronavirus is going to have on our health system. I’ll start by discussing how the staffing of our hospital system works normally, and then consider the impact of coronavirus.

As a medical registrar, I have worked in five different public hospitals in NSW, and as a general rule, while we manage, these hospitals, and their staff, are very busy. It is very unusual for us ward doctors to finish on time, even normally, and on average most doctors will probably finish between one and three hours later than our official finish time. We are used to this, and some will be paid overtime for these hours, but many of us do not. Often, we may go through the entire day without eating or going to the bathroom until 5 or 6 PM, sometimes later. At times I’ve had to do one or two sixteen hour shifts every week, get home at midnight, have dinner, shower, and go to bed, to wake again at 5.30 to be back at work at 7 the next morning for a slightly more tolerable 10-11 hour day.

Nurses may be slightly more likely to finish on time due to differences between the way doctors and nurses are rostered, but they tend to be run off their feet the entire time they are at work. They are their patients’ nurse, but that encompasses also being their carer, advocate, medical jargon interpreter, counsellor, educator, mobility aide, support, protector, comfort, adviser, and often family. Regularly, they may have to do either planned or unplanned double shifts due to staffing concerns.

Of course, the hospital system needs so much more than doctors and nurses to operate. There are our allied health colleagues - pharmacists, physiotherapists, occupational therapists, social workers, speech pathologists, dietitians, radiographers - our laboratory staff and scientists, support and administrative staff, porters, cleaners, orderlies, and volunteers. There are also of course all our executive staff and various other people. And I am extremely grateful to everyone of these people in all of our hospitals. Without any of them, the hospital would not be able to function. I would also like to give my sincere thanks and acknowledgement to all of my colleagues in the general practice world, doing great work in the community and looking after far more patients than we do in hospital, keeping so many people well and out of hospital, helping us out immeasurably.

Without naming names of hospitals, at most hospitals I have worked at, the hospital is perpetually either full or close to full. Hospitals generally have bed meetings with senior staff every day to discuss how to manage their patient load, and which patients can be discharged safely in order to accommodate more pressing and unwell patients in the emergency department. I have received countless pages (yes, we really do still use those beepy things) just after work starts along the lines of “HOSPITAL FULL PLEASE ATTEND TO PATIENT DISCHARGE URGENTLY”, as there might be 20 or more patients stuck in the emergency department needing admission, but with no free beds in the main hospital. We do generally manage this normal patient load, but not only is the hospital always functioning near capacity, high patient loads mean every single one of those staff members I have mentioned is quite busy.

When coronavirus hits, we are going to see two major issues affect the functioning of our hospitals. Firstly, patient numbers are going to skyrocket. In Italy, on march 28, they have over 80,000 cases, and over 9,000 dead. That mortality rate is over 10%! Much higher than most quoted figures of 1-2%. But this is a function of the overloaded hospital system. When everyone has access to ICU beds, and hospital staff that have time and resources to take outstanding care of patients, mortality rates will hopefully not exceed 1-2%. But when the number of patients that need ICU beds, ventilators, and highly trained staff to manage them massively exceeds the availability of those things, the majority of those people that can’t access them will die. This leads to huge increases in mortality as patients that could normally be saved, cannot.

Secondly, those key hospital staff that I’ve mentioned are going to start getting COVID-19. Healthcare workers are at far higher risk of contracting COVID-19 than the general public. In Italy, 20% of healthcare workers have been infected already, and that number continues to increase. These essential staff will be either quarantined at home for 14 days, or even worse, very unwell and requiring hospital care. This will both reduce our staffing at a time of great need, and increase the workload on those that remain.

Finally, I would like to remind everyone that hospitals will still need to be treating and looking after hundreds of other medical conditions. Whilst non-urgent elective surgeries will cease, the vast majority of conditions that normally occur in the population and need hospitalisation - they will all still be happening. People will continue to have strokes, heart attacks, pneumonias, COPD exacerbations, diabetic ketoacidosis and the like. But the hospital system overburdened with COVID-19 cases will also be managing all of these conditions and more.

So why am I telling you all this? At 3,500 cases as of today, we are just at the tip of the iceberg. Every individual’s actions make a difference. So I want to urge each and every one of you to do everything you can to prevent the spread of coronavirus. Cancel all non-essential outings. Work from home if you can. Avoid spending time with anyone you do not live with, unless it is in the capacity of essential work. Wash your hands with soap and water often, especially when leaving the house or getting back home. Practice physical distancing when you have to leave the home - stay at least 1.5 m away from other people, do not touch anyone.

To those of you that are still going to public events and putting everyone’s health in jeopardy, I have some questions for you:

Which family member or friend of yours would you be happy to die, if you are happy to risk anyone getting COVID-19?

If our cases continue to skyrocket, what if it’s you that has a car crash, or a ruptured appendix, or an aortic dissection. Do you want to be going to a hospital that is stretched far beyond capacity, or one where staff can give you the care and attention you need to get better?

If you are one of the unlucky ones, who needs a ventilator, do you want there to be one available? Or do you want to force some poor doctor to have to choose between saving your life and someone else’s.

As a wise woman once said “Our quest stands upon the edge of a knife. Stray but a little, and it will fail, to the ruin of all. Yet hope remains, while all the company is true.” We stand on the precipice of disaster, but we can still flatten the curve and prevent our health system being overwhelmed if each person takes responsibility for their own actions.

In Victoria, it is proven that bad builders have been able to get away with legal liabilities. In 2012, VBA’s operating procedures were exposed through the Victorian Ombudsman’s report. hello! love your blog! have a question not really pertaining to ext 2: is it true that you can get into any course at uni even if you don't get the entry atar. i know it takes a few extra years or something, heard of transfers and stuff like that. i looked it up but i thought i'd ask you because i don't understand all the uni jargon. haha thank you :)

I’d say it’s largely true. Australian universities nowadays understand that the ATAR isn’t the sole measure of academic performance and that students have more to offer than their grades, e.g. leadership, community involvement, work experience, etc. In all the major universities - Usyd, UNSW, WSU, Macquarie - there are pathway programs that cater to high achievers in non-academic areas and socially disadvantaged students.  

(Before I go anywhere, just a disclaimer that I’m not an expert on this and you should definitely consult the websites I’ve linked to get a clearer picture of how pathway programs and transfers work.)

University of Sydney

Early Offer Year 12 (E12) - for domestic NSW students attending a government-identified low socio-economic high school and/or are from a financially disadvantaged background. Comes with a $5000 scholarship and an iPad.  

Broadway - for domestic students who have experienced long term disadvantage that has impacted their academic performance in Year 11 and/or Year 12. It allows entry into most courses with an ATAR that is up to five points below the cut-off. 

Cadigal Alternative Entry - for Aboriginal and Torres Strait Islander people, whether they are in Year 12 or not.  

Elite Athletes and Performers Scheme - for those whose sporting commitments have significantly affected their academic results; will allow entry into courses with an ATAR of up to five points below the cut-off.

University of New South Wales 

Educational Access Scheme - same premise as Usyd’s Broadway program, but more widely applicable to Australian and NZ citizens or Australian permanent residents. 

Bonus Points - awards bonus points based on performance in relevant HSC subjects.

Macquarie University

Academic Entry Program - an early offer made on the basis of your performance in category A HSC courses that align with select Macquarie degrees. 

ATAR + bonus points - Macquarie offers five ways to add up to five bonus points to an ATAR: 1) Academic Advantage; 2) Rural Bonus Scheme; 3) Educational Access Scheme; 4) Athletes and Performers; 5) Big History Scheme.

Macquarie University International College - offers foundation programs and diplomas that on successful completion allow entry into first or second year respectively (applies only to certain degrees). Open only to high school students and non-school students without the necessary entry qualifications.

Next Step Pathway - for high school and non-school applicants who don’t meet the entry requirements for an undergrad degree. Involves studying four units part-time on campus.

Western Sydney University

Aboriginal and Torres Strait Islander Alternative Entry Program 

BMedicine/BSurgery - Rural Entry Admission Scheme 

Special Tertiary Admission Test - a multiple choice test administered by the UAC, whose results WSU accepts.

Western Sydney Advantage Entry Programs - includes bonus points, regional bonus points, and the Educational Access Scheme or Elite Athlete Performer Bonus Scheme.

Western Sydney University College - provides foundation programs and diplomas for local and overseas students that on successful completion open up access to university study. 

IMPORTANT: Make sure you understand which schemes require a separate application to the Universities Admission Centre (UAC) and which do not. In most cases you still need to apply to the UAC for university entry. Bonus points tend to be automatically calculated, but on the whole, alternative entry programs require a submission to the university of your choice once you’ve applied through the UAC.

Transfers

Generally there are two types: internal and from another university. Internal transfers are between degrees at the same university (conditions apply), the second type is self-explanatory. HOWEVER, transfer rules differ across the universities and you’ll need to do the necessary research in order to understand precisely what is required of you. The jargon will vary too, for example, Usyd has what are known as “allowable transfers” - transfers that are allowed by your faculty or school; UNSW has an Undergrad Internal Transfer Program (ITP). In some cases, e.g. transferring into a BMed/Surgery at WSU, you will have to go through the UAC. Transferring from one university to another will certainly require applying to the UAC first.

Common Transfer Paths

I know it’s very common for students wanting to do Med to firstly enrol in a Bachelor of Medical Science and then jostle for admission into Med proper (quite popular at UNSW). Some of my cohort actually ended up transferring to WSU for Med. 

In Law, students tend to enrol in a single degree with a lower ATAR, and then apply to transfer to a combined law degree after first year or even second year. Again, the process is competitive. At Usyd this is done through an internal transfer that takes into consideration your tertiary marks or secondary qualifications, whichever is higher. If you’re successful after first year you’ll be on track to complete an LLB in the standard five years.   

Wherever and however you transfer, you will need to read your faculties’ relevant handbooks to determine eligibility, deadlines, the exact process, your course progression post-transfer, and any additional units you might have to take. It’s also helpful to consult with academic advisers before you transfer. In any case there’s a lot of administrative footwork, so if you have your sights set on a certain degree you should do the research well in advance of deadlines.

COVID-19 news and trends

The numbers

Rising COVID-19 numbers in the US and Europe are alarming national leaders (in most cases) and leading to new lockdowns and predictions of winter case peaks.

In the US, cases are reported increasing in 41 states – the biggest surge since August – and insufficient testing may be contributing to under-reporting of new infections.

Across Europe, an average of more than 100,000 cases per day was reported in the past week. Yesterday France’s president Emmanuel Macron declared a state of emergency, including a 9pm–6am curfew starting on Saturday in nine cities.

The New York Times reported Macron as saying: “The virus is everywhere in France.”

On Monday, WHO director-general Tedros Adhanom Ghebreyesus told a media briefing: “Around the world, we’re now seeing an increase in the number of reported cases of COVID-19, especially in Europe and the Americas.

“Each of the last four days has been the highest number of cases reported so far. Many cities and countries are also reporting an increase in hospitalisations and intensive-care bed occupancy. At the same time, we must remember that this is an uneven pandemic. 

“Countries have responded differently, and countries have been affected differently. 

Almost 70% of all cases reported globally last week were from 10 countries, and almost half of all cases were from just three countries.”

Johns Hopkins University’s Centre of Systems Science and Engineering (CSSE) reported (at 15:00 AEST on Thursday 15 October) 38,441,934 confirmed cases and 1,091,439 deaths.

The Department of Heath reported on 7 October that national confirmed cases stood at 27,341, a rise of 25 in 24 hours. 904 deaths have been recorded.

Pause for safety – and definitions

Earlier this week US pharmaceutical giant Johnson & Johnson confirmed it was temporarily pausing its COVID-19 vaccine clinical trials due to an unexplained illness in a study participant. J&J did not elaborate on the illness, citing respect for the participant’s privacy.

In a statement, J&J stressed the priority it gives to “the safety and well being of the people we serve every day around the world”, and the difference between the terms “study pause” and a “regulatory hold”.

It says the former, “in which recruitment or dosing is paused by the study sponsor, is a standard component of a clinical trial protocol”. The latter “is a requirement by a regulatory health authority, such as the U.S. Food and Drug Administration (FDA). 

The study pause follows a similar study pause in September by British-Swedish pharmaceutical company AstraZeneca. Their trials have since resumed. 

Australian vaccine experts backed the company’s action and emphasised that such a pause isn’t unusual.

“It is not unusual to suspend a clinical trial if there is an adverse-reaction reported during testing,” wrote Murdoch University’s Jeremy Nicholson. “This is all part of checking that the vaccine (or drug) is safe. 

“People can fall ill in clinical trials by chance and it may have nothing to do with the vaccine itself, this is especially likely in a large trial. Each case has to be investigated thoroughly to evaluate the cause and likelihood of it being trial related.”

“It does not mean the trial will not continue, and is required under ethical standards governing clinical trials,” wrote Australian Catholic University’s Roger Lord.

More safety = less carbon

The COVID pandemic has profoundly affected human activities – a state of affairs that ought to be reflected in energy use and carbon dioxide (CO2) emissions. 

Now, an international team has presented daily estimates of country-level CO2 emissions for different sectors – for instance residential, transport and aviation – based on near-real-time activity data. The results have been published in Nature Communications.

The key result is an abrupt 8.8% decrease in global CO2 emissions in the first half of 2020 compared to the same period in 2019. This decrease is of a magnitude larger than during previous economic downturns or World War II. The timing of emissions decreases corresponds to lockdown measures in each country.

The research reveals that by 1 July, as lockdown restrictions relaxed and some economic activities restarted, especially in China and several European countries, the pandemic’s effects on global emissions diminished. But substantial differences persist between countries. In the US, for instance, where coronavirus cases are still increasing, emission declines have continued.

The study’s author’s write that the “absolute decreases in CO2 emissions are larger than any in history, including those that occurred during the recent 2008–2009 global financial crisis” and emphasise the message their results sends.

“At face value, an 8.8% relative reduction of emissions seems to be small when compared to the magnitude and extent of the disturbance of human activities that the COVID produced. This means that the long-term emissions decreases needed in this century to achieve [carbon reduction] targets must be based on structural and transformational changes in energy production systems, de-carbonisation of transportation and improved building energy use efficiency, that is an improvement of the carbon intensity of economies rather than decreases of human activities.”

Is SARS-CoV-2 here to stay?

Worldwide, only a few repeat SARS-CoV-2 infections have been verified since the pandemic began. But in a Perspective article in Science, Jeffrey Shaman and Marta Galanti suggest that it’s likely the virus will become endemic – that it will be able to re-infect humans who’ve had it before.

Shaman and Galanti write that in many respiratory viruses (such the flu and the common cold) a number of processes – including particularly insufficient adaptive immune response, waning immunity and immune escape – can allow subsequent reinfection. While many questions remain about the nature of these immune responses and trajectories in the case of SARS-CoV-2, insight from other respiratory viruses points to the possibility of reinfection with it.

Shaman and Galanti note that cyclic persistence of COVID in human populations may be affected by ongoing opportunities for interaction with other respiratory pathogens – it’s possible infection with a different virus could provide some short-lived protection to SARS-CoV-2.
