I arrive at a place that I never wanted to be.
Without warning, as often happens with cancer patients and their caregivers, everything in my life “changed on a dime.” It’s appropriate then, for my 1st entry into the Phoenix Cancer Support Network monthly blog, to give a short accounting as to how I became involved with this wonderful organization.

As a practicing gastroenterologist in the community, cancer screening, diagnosis and treatment was always a significant part of my day-to-day activities. Yet, even as a physician, I had very little insight into the issues and difficulties encountered by patients trying to navigate this unfamiliar and tortuous journey.
Then, 13 years ago my youngest daughter, Annie, away at college at the University of Arizona, began to complain of pain in her left leg. As there was a long history of low back problems in my family, we were concerned, but not overly alarmed, and accepted the pronouncement of the University Student Clinic that this was musculoskeletal and needed treatment with physical therapy and nonsteroidal anti-inflammatory drugs.
After this had continued for several weeks without relief, when the spring term ended, and she returned to Wisconsin, and ultimately, to summer camp counseling in North Carolina, I insisted that she first have an MRI of her low back. This was done at my hospital by a radiologist that I have known for 25 years and when the films were processed, we reviewed them together. We could see that the low back was normal but that there was “something unusual” in her left pelvic area.
From this point on, her life and those of our entire family were forever altered.
Even as a physician, familiar with medical practice, terminology and with the doctors, in a personal way, each step of the process of staging her disease, identifying its character and planning and executing a treatment plan were rife with road blocks, difficulties and missteps. Once treatment had begun, the process of juggling my work life, my daughter’s treatment, much of which was inpatient, and caring for my wife and other two children’s physical and emotional needs was extremely daunting and, at times, seemed impossible. We had no advisors, care managers or navigators to assist us. Mistake after mistake was made until we finally learned that we had to advocate for ourselves. There was no one out there to advise or lead us as we muddled, somehow, through the cancer maze.
Years later, after she had passed, we had to deal with the process of grief which has been ongoing now for the past 13 years.
I felt almost compelled to change my practice character, moving out of private gastroenterology practice in Milwaukee and joining the Cancer Treatment Centers of America in Goodyear, Arizona, devoting my career, perspiration and dedication to improving the lives of cancer patients on their own journeys.
When I was approached by my eldest daughter, Jeanette Martin, with her idea to create a nonprofit organization dedicated to easing the burden of the cancer journey, I was instantly enamored with the idea of helping relieve some of the very problems that my own family had encountered years before.
As PCSN approaches its first year of existence, I hear and see the relief from our clients own fears, concerns and worries as we offer and deliver the very kind of assistance that was absent for us and for Annie.
I question whether there is a greater calling or service that I ever would have been able to encounter or deliver and it has been my great privilege to participate with our wonderful CEO, my own daughter, the other incredible board members and voluntary staff that have miraculously appeared to fill this gap in patient care.
My heart filled thanks goes out to Jenny Martin, Nancy Weber, Fred and Andrea Dias, Gary and Susy Sennett, Rob and Dom Wascher, Raphael Sheffield, Julie Xander and Marcy Laront for their dedication, enthusiasm and hard work.

Position Duties Patient Care Services 1)Provide patient care services for which he/she is clinically privileged, including but not limited to: a)Provide direct patient care services on an inpatient and outpatient basis b)Provide subspecialty consultations and follow-up.

The iconic sauce remains a family secret and a vital part of the landmark restaurant located on the honkytonk strip on lower Broadway in downtown Nashville. These days, however, the Barbeque King is a fan of a whole different kind of secret sauce.
“My secret sauce now is being motivated and compliant, and doing a daily exercise that allows me to swallow food, especially my delicious ribs,” he says.
In what he agrees is a cruel irony, Jack lost his ability to swallow in 2004, when he was diagnosed with tonsillar cancer. He underwent surgery and several rounds of chemotherapy and radiation therapy. Because scarring and strictures developed in his esophagus, “swallowing became a real challenge,” Jack says.

Jack’s only way to stay alive was to endure a feeding tube for a year. “I couldn’t swallow or taste or smell food during that time,” he says. “That year was pretty ugly. I wanted to live, so I could die of something else, because that was terrible.”
He had to undergo frequent esophageal dilation procedures at a Nashville hospital to keep his esophagus open.  He could only swallow small amounts of food, and he didn’t look forward to frequent hospital visits for the dilation procedures that required anesthesia.
“I was curious, hoping for an alternative, so I Googled around,” Jack explains. “I just knew there had to be a better way.”
To his surprise, up popped Mayo Clinic’s Arizona campus, where he saw a video of a patient doing self-dilation. “It can be done at home,” Jack says. Standing next to the patient in the video was “a tall, distinguished man with a bow tie and a great bedside manner,” he adds.  That man was David Fleischer, M.D., a physician in Mayo’s Department of Gastroenterology and Hepatology.
Jack wasted no time contacting Mayo Clinic, and in February 2016, he was in a Mayo exam room, pondering his possible options. To his surprise, “in walked the man from the video,” Jack says.  As he gazed at Jack’s chart, Dr. Fleischer, who had earned his medical degree at Vanderbilt University in Nashville, asked Jack if he possibly was “the” Barbeque King on Broadway in Nashville. The answer was yes.  He told Jack, “I love that place!”
“Dr. Fleischer told me he couldn’t believe I was here, but he was glad that I was, and he was sorry that I couldn’t enjoy my own barbeque, as so many others do.  It seems unfair,” Jack says.
Jack qualified for self-dilation and underwent training on how to do it. The program at Arizona’s campus has trained more than 50 patients on how to perform self-dilation. The program is staffed by Dr Fleischer, other esophageal specialists, and nurses Robert Spratley and Lori Goette.
Now, every morning, Jack is dutiful about sipping water and then carefully guiding a long, flexible tube down his throat and into his esophagus to open the pathway for solid food. What at first seemed to be an intimidating procedure is now a simple routine that takes Jack only 15 seconds.

“Mayo could not have been nicer to train me, step-by-step, and to transfer my files to Vanderbilt University in Nashville, where I continue my care,” Jack says.
Dr. Fleischer had told Jack that he needed to be “motivated and compliant” to qualify for self-dilation. “I told him, ‘Bingo, that’s me!'” Jack says. “That is what works, and that is the real secret sauce. I was determined to get my throat open and to sit down and enjoy my barbeque with my friends and family.”
Now 70, Jack is still active in the business. Over the New Year’s holiday in 2017, Dr. Fleischer and members of his family were invited to have lunch with Jack, his son Jay and his family — at Jack’s Bar-B-Que.
“It has to be an answer to our prayers for two men to find one another in such a way,” Jack says, referring to Dr. Fleischer. “I am now able to encourage others to take that step that I did.  It just may change your life, as it did mine.”
Now 13 years in remission, Jack is enjoying and thankful for every day.

  With his wife by his side and a team of experts at Mayo Clinic to help him through, Jim Heinl faced down a cancer recurrence and then went back to a life focused on family.
First diagnosed at age 16, Jim Heinl knows full well what it’s like to be forced to deal with cancer. But he’s never had to face it alone. Jim’s high school sweetheart and wife of 26 years, Tami, has been by his side through it all. And, when the cancer came back decades later, Jim had a team of specialists at Mayo Clinic to depend on, too.
“When your life’s at stake, that’s where you want to be,” Jim says. “That’s where everyone is fighting for you.”
In 1985, Jim learned he had a rare form of liver cancer called fibrolamellar hepatoma. Treatment involved removing two-thirds of his liver.  But that wasn’t enough. The disease returned four years later, and the recurrence required Jim to undergo a liver transplant.
For 25 years after that, all was well. Jim graduated from college in 1990, and he and Tami were married in 1991. Over the next nine years, the couple welcomed four children to their family. They were the center of Jim’s world.
“My favorite thing to do is spend time with my wife and kids,” he says. “I love making them laugh and hearing them laugh.”

But in 2014, Jim’s medical situation took a scary turn. Scans revealed several spots on his left lung, as well as a mass in his pancreas. Jim’s physician at Mayo Clinic’s Arizona campus, Thomas Byrne, M.D., in the Department of Gastroenterology and Hepatology, decided to take a closer look, and he wanted to get his colleagues involved, too. Dr. Byrne brought Jim’s case to a Mayo Clinic tumor board where experts discuss complex cases to determine the best approach for each patient. In Jim’s case, the group agreed surgery would be best.
On Oct. 30, 2014, Jim underwent surgery to remove four areas of cancer that had been identified in his pancreas and lung. Afterward, follow-up testing showed no signs of cancer, and none have appeared since then.
Looking back on the experience at Mayo Clinic, Tami is grateful not only for the care Jim received, but for his caregivers’ willingness to include her in the process.
“Mayo Clinic was absolutely incredible about making sure that they were not only listening to Jim, but listening to me as well, knowing we were a team,” she says. “I love how everyone there has so much energy, so much passion and so much knowledge. They walk into the room, and you are their main focus.”
Although he continues to have regular checkups to make sure he remains cancer-free, today the disease is no longer front and center in Jim’s life. Instead, he’s back to spending time with and traveling with his family. Jim’s also returned to having fun with his hobbies, including playing billiards, enjoying family game night, listening to music and rooting for his favorite college football team — the Ohio State Buckeyes.
