Most likely: Bertie was killed in a random incident or by a random soldier, his death was completely identical to thousands of others who died in the war and there were probably even identical deaths of other soldiers in the same day. Tim was also shot by a random soldier after his rampage because eventually someone had to take him down and his luck couldn't last forever. This is because war is full of forgotten faces and meaninglessness, seldom few ever stand out enough to be remembered. War is an endless cycle of loss.
Canon compliant: The Toy Soldier was the one to kill Bertie, and Tim has to live with that fact for the rest of his forced existence alongside it. That is, if he ever even learns the Toy Soldier was the one to do it and if the Toy Soldier even deemed killing Bertie significant enough to remember among the hundreds it killed in the war.
Improbable but hilarious: The Toy Soldier was the one to kill Bertie AND it was the one to take down Tim. This man's entire life and personality was determined by a murderous wooden doll.
